探索根治性前列腺切除联合手术去势在极高危前列腺癌治疗中的价值。  

Objectives of Study：

To explore the clinical value of radical prostatectomy combined with surgical castration in the treatment of very high-risk prostate cancer.

药物成份或治疗方案详述：

纳入标准：

1血清PSA、睾酮水平、直肠指检显示异常并在彩超定位下行前列腺穿刺活检术后病理确诊为前列腺癌患者; 2满足极高危前列腺癌定义的患者:肿瘤主要成分Gleason分级5级、活检4针以上Gleason评分≥8分、临床分期≥T3b,3项指标至少符合其中之一,且无远处转移; 3术前影像学资料(如盆腔MRI、增强CT或PET-CT)提示局部病变可切除; 4治疗前确诊为雄激素依赖型前列腺癌患者; 5既往未接受过放疗或者内分泌治疗,并且无内分泌治疗禁忌症; 6具有良好的依从性、治疗过程规范且随访资料完整的患者

Inclusion criteria

1.Patients pathologically diagnosed with prostate cancer following abnormal findings on serum PSA, testosterone levels, and digital rectal examination (DRE), and confirmed by transrectal ultrasound (TRUS)-guided prostate needle biopsy; 2.Patients meeting the criteria for very high-risk prostate cancer, defined as having any one of the following: a predominant Gleason pattern 5, >=4 biopsy cores with a Gleason score >=8, or a clinical stage of >=T3b, with no evidence of distant metastasis; 3.Preoperative imaging (e.g., pelvic MRI, contrast-enhanced CT, or PET-CT) indicating that the local lesion is resectable; 4.Patients diagnosed with androgen-dependent prostate cancer prior to treatment initiation; 5.No prior history of radiotherapy or endocrine therapy, and no contraindications to androgen deprivation therapy (ADT); 6.Patients with good compliance, standardized treatment protocols, and complete follow-up data

排除标准：

1前列腺组织学或细胞学病理结果显示为神经内分泌性前列腺癌(包括小细胞癌)的患者; 2存在第二原发肿瘤的患者; 3因活动性病毒性肝炎、其他慢性肝病或心肺功能不全而存在手术禁忌的患者; 4不遵循医嘱、不配合随访或随访资料不完整的患者; 5研究人员认为不适合参与研究的其他患者

Exclusion criteria：

1.Patients with histopathological or cytological evidence of neuroendocrine prostate cancer, including small cell carcinoma; 2.Patients diagnosed with a second primary malignancy; 3.Patients with contraindications to surgery due to active viral hepatitis, other chronic liver diseases, or cardiopulmonary insufficiency; 4.Patients who fail to adhere to medical instructions, are uncooperative with follow-up, or have incomplete follow-up data; 5.Any other patients deemed unsuitable for study participation by the investigators.
