研究目的：

1.基于恐惧-回避模型探索老年髋部骨折术后跌倒恐惧发生现状及影响因素，应用结构方程模型方程探索跌倒恐惧的触发机制。 2.以认知行为疗法为指导，构建阶梯式护理干预方案并验证其临床应用效果。  

Objectives of Study：

1. Based on the fear avoidance model, explore the current situation and influencing factors of postoperative fall fear in elderly patients with hip fractures, and apply structural equation modeling equations to explore the triggering mechanism of fall fear. 2. Guided by cognitive-behavioral therapy, construct a step-by-step nursing intervention plan and verify its clinical application effect.

纳入标准：

①单侧新鲜髋部骨折，符合髋部骨折的诊断标准； ②年龄≥60岁； ③首次行单侧髋部手术（内固定术、人工股骨头置换术及全髋关节置换术）治疗； ④采用单项提问法评价患者是否有跌倒恐惧，若有纳入本研究（单条目问题法）； ⑤具有正常的认知及沟通交流能力； ⑥自愿参加并签署知情同意书者。

Inclusion criteria

1 Unilateral fresh hip fracture meets the diagnostic criteria for hip fracture; 2 Age >= 60 years old; 3 First unilateral hip surgery (internal fixation, artificial femoral head replacement, and total hip replacement) treatment; 4 Use a single item questioning method to evaluate whether patients have a fear of falling, and if included in this study (single item questioning method); 5 Having normal cognitive and communication skills; 6 Those who voluntarily participate and sign the informed consent form.

排除标准：

①病理性髋部骨折； ②偏瘫、帕金森病、认知障碍及精神疾病等影响髋部活动患者； ③多发创伤； ④视力、听力严重障碍者； ⑤曾接受髋关节置换术或髋部其他手术。

Exclusion criteria：

1 Pathological hip fracture; 2 Patients with hemiplegia, Parkinson's disease, cognitive impairment, and psychiatric disorders that affect hip activity; 3 Multiple traumas; 4 Individuals with severe visual and hearing impairments; 5 Has undergone hip replacement surgery or other hip surgeries.

在对照组常规护理基础上，实施老年髋部骨折术后跌倒恐惧阶梯式护理干预方案： 第一阶梯（术后1~3天），干预目的是纠正消极心理因素，干预内容是引导式暴露+认知重建，干预形式为线下个体干预，干预人员为骨科专科护士，干预时间为15~20分钟，干预次数为1次，如干预有效（同时满足跌倒效能量表＜11分和单条目问题“您是否因为恐惧而限制活动”回答为“否”），则停止干预，如干预无效，则进入第二阶梯； 第二阶梯（术后4天~出院前1天）：干预目的是纠正消极心理因素+行为干预，干预内容为行为干预+巩固认知，干预形式为线下个体干预，干预人员为骨科专科护士+骨科康复治疗师，干预时间为25~30分钟，干预次数为2次，分别在术后3天开展行为干预（骨科康复治疗师）及出院前1天开展巩固认知干预（骨科专科护士），如干预有效（同时满足跌倒效能量表＜11分和单条目问题“您是否因为恐惧而限制活动”回答为“否”），则停止干预，如干预无效，则进入第三阶梯； 第三阶梯（出院后1天~术后4周）：干预目的纠正消极心理因素+行为干预+汲取社会支持，干预内容为社会支持+预防复发+安全救护，干预形式为微信一对一电话干预，干预人员为骨科专科护士+心理咨询师，干预时间为35~40分钟，干预次数为3次，每周1个主题，分别是开展社会支持干预（骨科专科护士）、预防复发（心理咨询师）、安全救护（骨科专科护士），每次45~60 min。

The intervention content is guided exposure+cognitive reconstruction. The intervention form is offline individual intervention, and the intervention personnel are orthopedic specialist nurses. The intervention time is 15-20 minutes, and the intervention frequency is once. If the intervention is effective (while meeting the criteria of falling efficacy energy table<11 points and the answer to the single item question "Are you restricting activities due to fear" is "no"), the intervention will be stopped. If the intervention is ineffective, the second step will be entered; The second step (4 days after surgery to 1 day before discharge): The purpose of the intervention is to correct negative psychological factors and behavioral intervention. The intervention content includes behavioral intervention and cognitive consolidation. The intervention form is offline individual intervention, and the intervention personnel are orthopedic specialist nurses and orthopedic rehabilitation therapists. The intervention time is 25-30 minutes, and the intervention frequency is 2 times. Behavioral intervention (orthopedic rehabilitation therapist) is carried out 3 days after surgery, and cognitive consolidation intervention (orthopedic specialist nurse) is carried out 1 day before discharge. If the intervention is effective (while meeting the criteria of a fall efficacy score of less than 11 and the answer to the single item question "Are you restricting activities due to fear" is "no"), the intervention will be stopped. If the intervention is ineffective, the third step will be entered; The third step (1 day after discharge to 4 weeks after surgery): Intervention purpose: Correcting negative psychological factors, behavioral intervention, and drawing social support. The intervention content includes social support, preventing recurrence, and safe rescue. The intervention form is one-on-one phone intervention on WeChat, with orthopedic specialist nurses and psychological counselors as intervention personnel. The intervention time is 35-40 minutes, with 3 times of intervention. There is one theme per week, which is to carry out social support intervention (orthopedic specialist nurses), recurrence prevention (psychological counselors), and safe rescue (orthopedic specialist nurses), with each session lasting 45-60 minutes.
