研究目的:

椎体骨折是老年人最常见的骨质疏松性骨折,也是再发骨折的独立危险因素,其死亡率高后遗症多,严重影响老年人的生活质量。由于早期症状隐匿,椎体压缩性骨折容易被忽略。传统的双能 X 线骨密度仪由于老年椎体退变脊柱侧弯等导致假性增高会误判椎体骨折风险。FRAX@骨折风险评估工具由于骨折因素未完全考虑老年人特点和中国人群的数据则低估中国人群骨折风险。因此,椎体骨折风险的早期预警一直是临床中的难点。本研究利用老年人既往由于各种病症接受过临床 CT 检查已有的图像获得椎体 CT 值,与前期研究得出的骨折危险因素问卷和临床电子病历相结合,通过机器学习的算法,拟建立简便快捷较全面的老年骨质疏松性椎体骨折的预测模型。并将模型工具嵌入体检系统模块,进行前瞻性队列的验证。以期建立适合中国老年人群的椎体骨质疏松性骨折风险评估工具,并在体检人群中推广应用,达到早筛早治,降低椎体骨折率,提高老年人生活质量。  

Objectives of Study:

Vertebral fracture is the most common osteoporotic fracture in the elderly, and it is also an independent risk factor for recurrent fracture, with high mortality and many sequelae, which seriously affects the quality of life of the elderly.Because the early symptoms are occult, vertebral compression fractures are easily overlooked.Conventional dual-energy X-ray bone densitometers may misjudge the risk of vertebral fracture due to false increases in senile vertebral degeneration and scoliosis.The FRAX@ fracture risk assessment tool underestimated the fracture risk of the Chinese population because fracture factors did not fully consider the characteristics of the elderly and the data of the Chinese population.Therefore, early warning of vertebral fracture risk has always been a clinical difficulty.In this study, vertebral CT values were obtained by using existing images of the elderly who had undergone clinical CT examination due to various diseases, and combined with fracture risk factor questionnaires obtained from previous studies and clinical electronic medical records, a simple, fast and comprehensive prediction model for osteoporotic vertebral fractures in the elderly was intended to be established by machine learning algorithm.The model tool is embedded into the physical examination system module to verify the prospective cohort.It is expected to establish a risk assessment tool for vertebral osteoporotic fracture suitable for the elderly population in China, and promot

纳入标准:

1.由影像学资料诊断新发骨质疏松性椎体骨折; 2.椎体骨折节段为 T11-L2; 3.年龄>=60 岁; 4.可获取完整的椎体 CT 影像学资料; 5.有研究所需完整的临床和老年综合评估的资料。

Inclusion criteria

1. Diagnosis of new osteoporotic vertebral fracture by imaging data; 2. Vertebral fracture segment is T11-L2; 3. Age>=60 years old; 4. Complete vertebral CT imaging data can be obtained; 5. Have complete clinical and geriatric comprehensive assessment data required for research.

排除标准:

1.暴力性骨折、脊柱转移瘤或原发性肿瘤、脊柱感染者; 2.胸腰椎内固定和/或融合手术史,椎体强化手术史; 3.由于严重脊柱侧弯及椎体旋转影响感兴趣区勾画者; 4.图像质量差不满足组学特征提取要求者。

Exclusion criteria:

1. Violent fractures, spinal metastases or primary tumors, and spinal infections; 2. History of thoracolumbar internal fixation and/or fusion surgery, history of vertebral body strengthening surgery; 3. Those who affect the delineation of the area of interest due to severe scoliosis and vertebral rotation; 4. Poor image quality does not meet the requirements of omics feature extraction.
