研究目的：

1. 系统分析富含热休克蛋白肽的外囊泡在 50 例肩周炎患者中的临床疗效，明确其对疼痛缓解、肩关节活动度改善的具体效果，量化炎症指标（如 IL-6、TNF-α）的变化趋势，评估治疗安全性。 2. 建立析富含热休克蛋白肽的外囊泡应用于肩周炎治疗的规范化观察体系，为其临床推广提供真实世界证据，探索不同病程、年龄患者的差异化疗效特征。  

Objectives of Study：

1. Evaluate the clinical efficacy of exosomes enriched with heat shock protein peptides in 50 patients with periarthritis of the shoulder, to clarify their specific effects on pain relief and improvement of shoulder joint range of motion, quantify the trend of changes in inflammatory markers (such as IL-6, TNF-α), and assess treatment safety. 2. Establish a standardized observation system for the application of exosomes enriched with heat shock protein peptides in the treatment of periarthritis of the shoulder, providing real-world evidence for its clinical promotion, and explore differentiated efficacy characteristics among patients with varying disease durations and ages.

纳入标准：

符合《康复医学》第 5 版中肩周炎诊断标准；年龄 18-70 岁；病程≥1 个月；常规肩关节 X 线摄片基本正常；自愿参与并签署知情同意书

Inclusion criteria

Meets the diagnostic criteria for periarthritis of shoulder in the 5th edition of Rehabilitation Medicine; aged 18-70 years; disease duration >= 1 month; routine shoulder joint X-ray examination basically normal; voluntarily participate and sign informed consent.

排除标准：

近 1 个月内接受过肩部手术或注射治疗；合并肩袖损伤等其他严重肩部疾病；孕妇或哺乳期妇女；合并骨折、结核等疾病；合并严重脏器功能不全、糖尿病等；对富含热休克蛋白肽的外囊泡或相关药物过敏；患有自身免疫性疾病或代谢性疾病；存在精神障碍或认知功能障碍。

Exclusion criteria：

Within the past month, have undergone shoulder surgery or injection therapy; combined with other severe shoulder diseases such as rotator cuff tears; pregnant women or nursing mothers; combined with fractures, tuberculosis and other diseases; combined with severe organ dysfunction, diabetes, etc.; allergic to exosomes rich in heat shock protein peptides or related drugs; suffering from autoimmune diseases or metabolic diseases; presence of mental disorders or cognitive function disorders.
