研究目的：

1、探寻心脏手术病人术前、术中体内炎症因子与术后膈肌功能障碍是否有相关性？其是否能用于预测早期心脏术后的膈肌功能障碍？ 2、探寻术前、术中相同指标在不同时间点的差异与膈肌功能障碍的关系 3、确定炎症敏感指标及其临界值用于预测心脏手术患者发生术后膈肌功能障碍风险  

Objectives of Study：

1. Is there a correlation between preoperative and intraoperative inflammatory factors and postoperative diaphragmatic dysfunction in patients undergoing cardiac surgery? Can it be used to predict diaphragmatic dysfunction after early cardiac surgery? 2. Exploring the relationship between differences in preoperative and intraoperative identical indicators at different time points and diaphragmatic dysfunction 3. Determine inflammation sensitive indicators and their critical values for predicting the risk of postoperative diaphragmatic dysfunction in patients undergoing cardiac surgery

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.术前超声显示膈肌偏移度及膈肌增厚率在正常范围：1）DE 女性＞9mm，男性＞10mm。 2.年龄＞18岁 3.在我院行体外循环心脏手术的患者

Inclusion criteria

1. Preoperative ultrasound showed that the deviation and thickening rate of the diaphragm were within the normal range: 1) DE female>9mm, male>10mm; 2. Age>18 years old; 3. Patients undergoing extracorporeal circulation cardiac surgery in our hospital.

排除标准：

1.患有重症肌无力、肌无力综合症、周期性瘫痪、多发性肌炎等神经肌肉病史； 2.既往开胸、气胸； 3.BMI>35； 4.术后皮下气肿明显影响超声成像检查者； 5.胸前区有大面积烧伤疤痕或既往有胸部伤口、乳腺根治术等影响膈肌超声检查效果者； 6.患者及家属拒绝签署书面知情同意书； 7.因合并精神病史、认知功能障碍等原因患者不能配合完成实验者；

Exclusion criteria：

1. History of neuromuscular disorders such as myasthenia gravis, myasthenia gravis syndrome, periodic paralysis, and polymyositis; 2. Previous thoracotomy and pneumothorax; 3.BMI>35; 4. Postoperative subcutaneous emphysema significantly affects ultrasound imaging examination; 5. Patients with extensive burns and scars in the chest area or previous chest wounds, mastectomy, etc. that affect the effectiveness of diaphragmatic ultrasound examination; 6. Patients and their families refuse to sign a written informed consent form; 7. Patients who have a history of mental illness or cognitive impairment are unable to cooperate with the experimenter;
