研究目的：

观察SGLT2i对肥胖2型糖尿病肾病早期筛查患者的临床疗效。  

Objectives of Study：

To observe the clinical efficacy of SGLT2i in obese patients with early screening of type 2 diabetic nephropathy.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①明确诊断为T2DM及DKD； ②基线体重指数（BMI）28.0-40.0kg/m2合并腹型肥胖（腰围男≥90cm, 女≥85cm）； ③年龄18-75岁； ④病程0-5年； ⑤基线糖化血红蛋白（HbA1c）≥6.5%； ⑥非初发患者稳定剂量的降糖药治疗至少8周。

Inclusion criteria

1. Clearly diagnosed with type 2 diabetes mellitus (T2DM) and diabetic kidney disease (DKD); 2. Baseline body mass index (BMI) 28.0 - 40.0 kg/m^2 with abdominal obesity (waist circumference >= 90 cm for men and ≥ 85 cm for women); 3. Age 18 - 75 years old; 4. Disease duration 0 - 5 years; 5. Baseline glycated hemoglobin (HbA1c) >= 6.5%; 6. Non-na?ve patients have been treated with stable doses of hypoglycemic drugs for at least 8 weeks.

排除标准：

①排除其他原因所致的慢性肾脏病； ②妊娠、拟妊娠或哺乳期女性； ③近6个月内出现过糖尿病急性并发症如DKA或HHS； ④近三个月有用过GLP-1受体激动剂及减重药； ⑤患有急性或慢性肝病，或筛选前4周内连续2次检查转氨酶（ALT或AST）大于正常范围上限3倍及以上，重度肾功能不全或终末期肾病； ⑥筛选前3个月内有显著心脑血管事件：充血性心衰达到NYHA分级III级或IV级、心肌梗死、TIA、脑梗死等； ⑦有反复泌尿道感染病史； ⑧半年内有酮症酸中毒病史。

Exclusion criteria：

1. Exclusion of chronic kidney disease caused by other reasons; 2. Pregnant, planning to be pregnant or lactating women; 3. Acute complications of diabetes such as DKA or HHS occurred within the last 6 months; 4. Use of GLP-1 receptor agonists or weight loss drugs within the last 3 months; 5. Acute or chronic liver disease, or two consecutive tests of transaminase (ALT or AST) within the upper limit of the normal range 3 times or more within 4 weeks before screening, severe renal insufficiency or end-stage renal disease; 6. Significant cardiovascular and cerebrovascular events within the last 3 months before screening: congestive heart failure reaching NYHA grade III or IV, myocardial infarction, TIA, cerebral infarction, etc.; 7. History of recurrent urinary tract infections; 8. History of ketoacidosis within the last half year.
