研究目的：

慢性鼻窦炎是发生于鼻窦黏膜的慢性炎症性疾病，患者主要表现出鼻塞、流涕、头面部胀痛和嗅觉功能障碍等临床症状，部分患者还可出现打鼾和耳部症状，病情通常较为复杂，治愈难度大，重影响患者生活质量。鼻内镜手术是目前临床治疗慢性鼻窦炎的有效方式，然而由于鼻部交感及副交感神经分布较为丰富，对于手术过程造成的机械刺激及术后术腔填塞的胀痛较为敏感，患者术中及术后常出现较为剧烈的应激反应，进而产生明显疼痛感。术后疼痛是鼻内镜手术中最复杂的问题之一。本研究针对鼻内镜手术，与传统气管插管全身麻醉相比，评估超声引导下蝶腭神经节阻滞对鼻内镜手术患者的术后疼痛的临床效果。优化目前鼻内镜手术的镇痛方案，减少阿片类药物使用，加速患者术后康复。  

Objectives of Study：

Chronic sinusitis is a chronic inflammatory disease affecting the nasal sinus mucosa. Patients primarily present with clinical symptoms such as nasal congestion, rhinorrhea, facial pressure and pain, and olfactory dysfunction. Some may also experience snoring and ear-related symptoms. The condition is often complex, difficult to cure, and significantly impacts patients' quality of life. Endoscopic sinus surgery (ESS) is currently an effective clinical treatment for chronic sinusitis. However, due to the rich distribution of sympathetic and parasympathetic nerves in the nasal cavity, patients are highly sensitive to mechanical stimulation during surgery and postoperative packing-induced pain, often leading to intense stress responses and significant pain both during and after the procedure. Postoperative pain remains one of the most challenging issues in ESS. This study focuses on ESS and, in comparison with traditional general anesthesia using endotracheal intubation, evaluates the clinical efficacy of ultrasound-guided sphenopalatine ganglion block in alleviating postoperative pain for patients undergoing ESS. The aim is to optimize current analgesic protocols for ESS, reduce opioid use, and accelerate postoperative recovery.

纳入标准：

1.65 岁?>=年龄?>=18 岁； 2.均符合 2018 版《中国慢性鼻窦炎诊断和治疗指南》中慢性鼻窦炎相关诊断标准， 并经 CT 或 MRI 检查证实 3.经保守治疗无效； 4.首次手术治疗； 5.ASA（美国麻醉医师协会麻醉评分）分级为Ⅰ～Ⅱ级； 6.自愿接受本研究治疗方式。

Inclusion criteria

1.65 years >=age >=18 years; 2. All meet the diagnostic criteria related to chronic sinusitis in the 2018 edition of the "Chinese Guidelines for the Diagnosis and Treatment of Chronic Sinusitis", and confirmed by CT or MRI examination 3. Conservative treatment is ineffective; 4. First surgical treatment; 5. ASA (American Society of Anesthesiologists Anesthesiologist Score) grade I.~II. 6. Voluntarily accept the treatment modality of this study.

排除标准：

1.对本研究涉及药物过敏者； 2.面部皮疹，感染，影响超声引导下颧下入路蝶腭神经节阻滞操作者； 3.未经控制或控制效果不佳的基础性疾病患者； 4.肝肾功能不全者； 5.心理及精神疾病患者； 6.不愿参加本实验者；

Exclusion criteria：

1. Those who are allergic to the drugs involved in this study; 2. Facial rash, infection, affecting ultrasound-guided subzygomatic approach sphenopalatal ganglion block operators; 3. Patients with uncontrolled or poorly controlled underlying diseases; 4. Patients with liver and kidney insufficiency; 5. Patients with psychological and mental illness; 6. Those who are unwilling to participate in this experiment;

随机方法（请说明由何人用什么方法产生随机序列）：

由研究者使用SPSS软件生成2个随机数字表，根据随机数将接受心脏外科手术的患儿分配为2组：1.常规全身麻醉组2.蝶腭神经节阻滞组。将随机化结果在麻醉前告知操作麻醉医生进行干预，在干预后在由不知分组情况的另外一名麻醉医生进行麻醉管理。

Randomization Procedure (please state who generates the random number sequence and by what method)：

The investigator generated two randomization schedules using SPSS software. Based on the random numbers, pediatric patients undergoing cardiac surgery were allocated into two groups: 1) conventional general anesthesia group, and 2) sphenopalatine ganglion block group. The randomization results were disclosed to the attending anesthesiologist prior to anesthesia for intervention implementation. Following the intervention, subsequent anesthesia management was performed by a separate anesthesiologist who was blinded to the group allocation.
