研究目的：

构建以患者“院前-院中-院后”为时间轴，贯穿患者在医疗机构门诊就诊、住院治疗、康复出院等多个环节的多专科课程体系，在各医疗机构推广应用。  

Objectives of Study：

Construct a multi-specialty curriculum system with the time axis of "pre-hospital - in-hospital - post-hospital" for patients, covering multiple links such as outpatient visits, inpatient treatment, rehabilitation and discharge of patients in medical institutions, and promote and apply it in various medical institutions.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1、患者纳入标准：（1）年龄＞=18 岁；（2）神志清楚，无沟通障碍，能理解互联网宣教意义，自愿参加本研究；（3）无视觉、听觉障碍，无声、光及动画演示过敏或错觉，拥有并能使用智能手机、具备操作微信软件能力的患者。 2、主要照顾者纳入标准：（1）在患者患病、治疗及康复期间，均全程参与的家属照顾者；（2）有一定的表达能力。 3、医护人员纳入标准：（1）参与患者的治疗及护理的医务人员；（2）有2年以上的工作经验。

Inclusion criteria

1.Inclusion criteria for patients: (1) Age ≥18 years old; (2) Be mentally clear, have no communication barriers, be able to understand the significance of online evangelism, and be willing to participate in this research. (3) Patients without visual or auditory impairments, who have no allergic reactions or illusions to sound, light or animation demonstrations, and who own and can use a smart phone and have the ability to operate wechat software. 2.Inclusion criteria for primary caregivers: (1) Family caregivers who have been fully involved throughout the patient's illness, treatment and rehabilitation period; (2) Have a certain ability of expression. 3.Inclusion criteria for medical staff: (1) Medical staff involved in the treatment and care of patients; (2) Have more than 2 years of working experience.

排除标准：

1、患者排除标准：（1）认知功能障碍者；（2）严重全身性疾病者。 2、主要照顾者排除标准：（1）不愿意参加本研究者；（2）中途退出照顾的人员。 3、医护人员排除标准：不愿意参加本研究者。

Exclusion criteria：

1.Patient exclusion criteria:(1) Individuals with cognitive dysfunction; (2) Those with severe systemic diseases. 2.Exclusion criteria for primary caregivers: (1) Unwillingness to participate in this study; (2) Personnel who withdraw from care halfway. 3.Exclusion criteria for medical staff:Unwillingness to participate in this study.

每次治疗或检查时，责任护士对患者进行相关指导，每天责任护士实施床旁的口头健康宣教，发放宣教资料。定期开展患者宣教会，通过PPT讲课方式为患者和家属授课，集中向患者讲解疾病相关知识。
