研究目的:

不可避免的缺血再灌注损伤(IRI)导致移植肝功能低下或无功能,无法发挥核心代谢器官的相关作用,严重影响患者预后。肝脏是机体最为活跃的代谢器官之一,能量代谢状态对供肝损伤修复的影响鲜见报道。预实验和靶向代谢组学提示肝移植复流早期,供肝组织的氧化磷酸化供能并未恢复,仍以无氧糖酵解供能为主,导致乳酸含量居高不下,发生了以无氧氧化为主的能量代谢重编程,这可能是阻碍供肝组织早期修复的关键,但目前具体机制尚未可知。基于此,本研究通过大数据研究中心数据库,获取肝移植患者术后动脉血气乳酸含量与肝功能指标,通过队列分析,研究二者的相关性,验证乳酸是导致供肝组织早期修复延迟的关键原因,为促进移供肝损伤修复提供新靶点。  

Objectives of Study:

Inevitably, ischemia-reperfusion injury (IRI) leads to impaired or non-functional transplanted liver, which cannot perform the associated roles of a core metabolic organ and severely affects patient prognosis. The liver is one of the most active metabolic organs in the body, and reports on the impact of energy metabolic status on donor liver injury repair are rare. Preliminary experiments and targeted metabolomics suggest that early in the reperfusion process of liver transplantation, the oxidative phosphorylation energy supply of the donor liver tissue has not yet recovered, remaining primarily reliant on anaerobic glycolysis, resulting in persistently high levels of lactic acid. This may be a key factor hindering the early repair of donor liver tissue, yet the specific mechanism remains unknown. Based on this, this study utilizes the database from the Big Data Research Center to obtain post-operative arterial blood gas lactic acid levels and liver function indicators in liver transplant patients. Through cohort analysis, it investigates the correlation between the two, confirming that lactic acid is a key factor leading to the delay in early repair of donor liver tissue, providing a new target for promoting the repair of donor liver injury.

纳入标准:

1.因终末期肝病,如肝硬化、肝癌、急性肝衰竭等在我院行肝移植手术的患者。

Inclusion criteria

1.Patients with end-stage liver diseases, such as liver cirrhosis, liver cancer, and acute liver failure, who underwent liver transplantation surgery in our hospital.

排除标准:

1.术前合并其他严重心脏、肺脏、肾脏疾病的 ASAⅢ级及以上患者;
2.存在活动性感染:如HIV、结核等;
3.恶性肿瘤转移:存在肝外转移的恶性肿瘤;
4.药物滥用:长期酗酒或药物滥用且未戒断;

Exclusion criteria:

1.Patients with ASA grade Ⅲ or above, who have severe preoperative comorbidities involving the heart, lungs, and kidneys; 2.Patients with active infections, such as HIV or tuberculosis; 3.Malignant tumors with metastasis: Malignant tumors with extrahepatic metastasis; 4.Substance abuse: Long-term alcohol abuse or drug abuse without abstinence.
