研究目的：

基于GUSS量表摸索一套胃管拔管前吞咽功能恢复情况的评估策略，及时评估患者，减少或预防急性脑卒中患者误吸的发生，减少并发症、提高患者的生存质量。  

Objectives of Study：

Based on GUSS scale, we explored a set of evaluation strategies for the recovery of swallowing function before gastric tube extubation, evaluated patients in time, reduced or prevented the occurrence of aspiration in acute stroke patients, reduced complications and improved the quality of life of patients.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

经CT或MRI确诊为脑卒中且符合《各类脑血管疾病诊断要点》中对脑卒中的诊断标准；胃管植入患者；年龄18~70岁；患者和家属知情同意。

Inclusion criteria

The patient was diagnosed with stroke by CT or MRI and met the diagnostic criteria for stroke in the Diagnostic Essentials of Various Cerebrovascular Diseases. Patients with gastric tube implantation; Age 18-70 years old; Informed consent of patient and family.

排除标准：

气管切开和气管插管患者；非口鼻胃管植入患者；非首次卒中患者。

Exclusion criteria：

Patients with tracheotomy and tracheal intubation; Non-oral-nasogastric tube implantation patients; Non-first-time stroke patients.

对留置胃管患者每日进行NIHSS评分，对于NIHSS评分<15分的患者进行GUSS吞咽评估，当患者GUSS评分>=15分时拔管，拔管后仍定期评估，调整进食性状，加强进食管理同时结合吞咽训练，促进吞咽功能的恢复。NIHSS评分>=15分时继续保留胃管，每日动态评估NIHSS评分，直至有条件进入GUSS量表评估。

干预措施代码：

Intervention：

Patients with an indwelling gastric tube were evaluated by NIHSS daily, and those with NIHSS score < 15 underwent GUSS swallowing assessment. When patients' GUSS score was ≥ 15, the gastric tube was removed. After extubation, regular assessments were continued, dietary consistency was adjusted, dietary management was strengthened, and swallowing training was implemented to promote recovery of swallowing function. For patients with NIHSS score ≥ 15, the gastric tube was retained, and NIHSS scores were dynamically assessed daily until conditions allowed for inclusion in the GUSS scale assessment.

对留置胃管患者每日进行洼田饮水试验，当患者洼田饮水试验为Ⅲ-Ⅳ级时，对患者进行摄食能力评估：给与200ml糊状饮食，如患者能吃完，并重复两次试验且均通过，则拔除胃管。

Intervention：

The patients with indurated gastric tube were subjected to a lowa drinking water test every day. When the patients' lowa drinking water test was Ⅲ-Ⅳ, the feeding capacity of the patients was assessed: 200ml paste diet was given. If the patients could eat it and repeated two tests and both passed, the gastric tube was removed.
