研究目的：

探讨星状神经节阻滞是否能改善体外循环下心脏手术患者术后肺部氧合功能。  

Objectives of Study：

To explore if stellate ganglion block improves postoperative pulmonary oxygenation in cardiac surgery patients with cardiopulmonary bypass.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

(1)年龄＞18岁； (2)ASA Ⅰ~III级； (3)择期体外循环下行开放式心脏瓣膜或冠脉手术； (4)自愿签署知情同意书。

Inclusion criteria

1.Age > 18 years; 2.ASA physical status classification I to III; 3.Elective open cardiac valve or coronary artery surgery under cardiopulmonary bypass (CPB); 4.Voluntarily signed informed consent.

排除标准：

(1)BMI≥30kg/m2； (2)术前脉搏氧饱和度＜95%； (3)左心功能不全； (4)涉及深低温停循环的手术； (5)感染性心内膜炎或其他感染的证据； (6)既往心脏手术或肺部手术史； (7)合并 COPD、肺纤维化、哮喘或其他慢性肺部疾病； (8)术前1个月内存在肺部感染或败血症； (9)术前存在中重度贫血； (10)术前存在肝硬化； (11)合并神经肌肉疾病（胸腺瘤、肌无力、肌病、肌营养不良等）； (12)对局部麻醉药过敏； (13)穿刺部位感染； (14)凝血功能障碍； (15)对侧膈神经或喉返神经麻痹； (16)青光眼； (17)精神障碍不配合者。

Exclusion criteria：

1.Body mass index (BMI) >= 30 kg/m2; 2.Preoperative pulse oxygen saturation < 95%; 3.Left ventricular dysfunction; 4.Procedures involving deep hypothermic circulatory arrest; 5.Infective endocarditis or evidence of other active infections; 6.History of cardiac or pulmonary surgery; 7.Comorbid chronic pulmonary diseases, including COPD, pulmonary fibrosis, asthma, etc.; 8.Pulmonary infection or sepsis within 1 month before surgery; 9.Moderate to severe anemia prior to surgery; 10.Preoperative liver cirrhosis; 11.Neuromuscular disorders (e.g., thymoma, myasthenia gravis, myopathy, muscular dystrophy); 12.Allergy to local anesthetics; 13.Infection at the puncture site; 14.Coagulopathy; 15.Contralateral phrenic nerve or recurrent laryngeal nerve paralysis; 16.Glaucoma; 17.Psychiatric disorders that affect cooperation.

由非盲统计师采用 SPSS 26 产生随机数，试验组：对照组的病例数比例为 1:1，总例数为116例。

The randomization sequence was generated by an independent, unblinded statistician using SPSS version 26, with a 1:1 allocation ratio between the SGB group and the control group. The planned total sample size was 116 participants.

本研究为单盲研究，实施SGB的操作人员对分组知情，从随机化开始直至数据库锁定为止，受试者、术中麻醉管理者、术后随访人员对分组保持盲态。

Blinding：

This is a single-blinded study. The personnel performing the stellate ganglion block are unblinded to group allocation. However, the participants, anesthesiologists responsible for intraoperative management, and postoperative outcome assessors remain blinded from randomization through to database lock.
