研究目的：

通过开展多中心临床病例-对照研究，进一步验证重金属、PAHs 等环境污染物暴露与 IMN 发生发展的相关性，为肠道菌群及其代谢产物在环境污染物影响 IMN 发生发展的作用提供人群关联证据。  

Objectives of Study：

A multi-center clinical case-control study was conducted to further verify the correlation between exposure to environmental pollutants such as heavy metals and PAHs and the occurrence and development of IMN, providing population-related evidence for the role of intestinal flora and its metabolites in the occurrence and development of IMN affected by environmental pollutants.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

IMN病例组： 1、初次发病，肾活检病理诊断符合IMN的诊断标准患者； 2、既往未使用过激素或其他免疫抑制剂治疗； 3、PLA2R抗体阳性； 4、年龄在18-70岁； 5、患者知情同意。 健康对照组： 1、无自身免疫疾病史、恶性肿瘤、高血压、糖尿病等慢性疾病； 2、近3个月内未罹患任何胃肠道相关疾病； 3、年龄在18-75岁； 4、性别、年龄等与病例组相匹配； 5、患者知情同意。

Inclusion criteria

Inclusion criteria for case group: 1 Patients with first onset and pathological diagnosis of kidney biopsy meeting the diagnostic criteria of IMN; 2 No previous use of hormones or other immunosuppressive therapy; 3 PLA2R antibody positive; 4 Age 18-75 years old; 5 Patient informed consent. Control group inclusion criteria: 1 No history of autoimmune diseases, malignant tumors, hypertension, diabetes and other chronic diseases; 2 no gastrointestinal diseases in the past 3 months; 3 Aged 18-70 years old; 4 Gender and age were matched with the case group;5Patient informed consen 5 Patient informed consent.

排除标准：

IMN病例组： 1、患有自身免疫性疾病、继发性肾小球肾炎、合并糖尿病或胃肠道疾病患者； 2、在留取标本1个月内服用过抗生素治疗，益生菌/益生元或其他微生态制剂; 3、妊娠期和哺乳期妇女。 健康对照组： 1、患有冠心病、糖尿病、高血压及肿瘤等疾病史； 2、患有肾脏疾病及其他全身系统性疾病者； 3、近1个月内服用过益生菌/益生元或其他微生态制剂。

Exclusion criteria：

Exclusion criteria for case groups 1 Patients with autoimmune diseases, secondary glomerulonephritis, combined diabetes or gastrointestinal diseases 2 Have taken antibiotics, probiotics/prebiotics or other microecological preparations within 1 month of specimen collection; 3 Pregnant and lactating women.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

数据采集：本研究采用电子化数据记录系统，通过电子病历和电话随访收集受试者的个人史、合并症病史、用药情况、肝肾功能、血常规等指标。 数据管理：数据将录入病例记录表（CRF），并使用电子数据采集和管理系统（如ResMan）进行管理。数据的收集与填写由研究者或其授权人员完成，确保数据的准确性和完整性。数据最终将导出至Excel表格，用于统计分析。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Data collection: This study adopted an electronic data recording system to collect subjects' personal history, comorbid history, drug use, liver and kidney function, blood routine and other indicators through electronic medical records and telephone follow-up. Data management: Data will be entered into the Case record Form (CRF) and managed using an electronic data acquisition and management system such as ResMan. Data collection and filling are completed by researchers or their authorized personnel to ensure the accuracy and completeness of data. The data will eventually be exported to Excel tables for statistical analysis.
