研究目的：

本队列建设项目核心目的是，采用“1+N”的模式，建立互利、公平、开放、共享、共进的中国妇幼健康与质量提升研究联盟（Research Alliance for Maternal-Child Health & Quality Improvement, ALL for MCH-QI）队列研究平台。具体阐述如下： （1）“1”-建立基础队列：基于国内非常完善的规律产检和住院分娩服务体系和高度信息化和结构化的病历记录，以最经济、高效的方式，整合社会人口特征、临床检查等信息，构建多中心大样本孕产妇回顾性队列，形成满足多病共研的全人群基础数据库，为产科疾病的监测、高危识别、预测，以及更多前瞻性队列和RCT研究的开展，提供基础数据支持； （2）“N”-拓展专题队列：在基础队列基础上，建立N支小型前瞻性专题队列，为一些特定问题和特殊疾病的深入研究（如发病机制研究）提供强有力的平台支持。 这一建设模式将充分利用现在医院信息化优势，以最经济高效的方式，满足多暴露多结局和专病/专题专研的队列建设需求。  

Objectives of Study：

The objective is to establish the Research Alliance for Maternal-Child Health & Quality Improvement (ALL for MCH-QI) with the principles of mutual benefit, fairness, openness, sharing, and collective progress, and to build a cohort research platform by adopting a "1+N" model. (1) "1" - Establishing a basic cohort: Based on the well-established system of regular antenatal and hospital delivery services in China, we aim to build a retrospective cohort of multi-center and large sample of pregnant women (100,000 cases during the project period), integrating the information on socio-demographic characteristics, clinical examinations, disease diagnosis and clinical practices. This cohort will form a population-wide basic database that meets the multimorbidity coresearch requirements, providing basic data support for real-world studies on surveillance, high-risk identification, prediction and interventions for obstetric diseases, as well as the conduct of more prospective cohorts and RCT studies. (2) "N" - Establishing N thematic cohorts: Based on the basic cohort, N prospective thematic cohorts will be established to provide strong platform support for the in-depth study of some specific issues and special diseases, such as pathogenesis study; Within the project period, a birth cohort will be established, enrolling 500 pregnant women and following up until their infants reach one year of age.

纳入标准：

1.回顾性队列：2018-2024年之间妊娠；孕妇怀孕时年龄18-49岁；在成员单位常规产检，首次产检小于14周，孕早、中、晚期各阶段至少一次产检；在所在成员单位分娩或终止妊娠。
2.前瞻性队列：2025-2027年之间妊娠；怀孕年龄在18-49岁之间；已在成员单位建档，当前孕周小于14周；计划在所在成员单位规律产检和分娩；并预期子代3岁以前儿童保健均在本研究中心完成；知情同意后愿意参与本研究。

Inclusion criteria

1.Retrospective cohort: pregnancy between 2018 and 2024; aged 18-49 years at the time of pregnancy; routine prenatal examination in member units, with the first prenatal examination less than 14 weeks, and at least one prenatal examination in first, second and third trimesters; delivered or underwent pregnancy termination at the member units.
2.Prospective cohort: Women who are pregnant between 2025 and 2027, aged 18 to 49, have been registered in the member units, and are currently less than 14 weeks of pregnancy; plan to undergo regular prenatal examinations and deliveries at their respective member units; all child health care for the offspring will be completed at this research center before the age of three years; provide informed consent and agree to participate in the study.

1.回顾性队列：排除所需关键信息项（详见下文）不完整者。
2.前瞻性队列：排除退出研究者，或因故去其他医院就诊，尝试多种措施无法随访者。

Exclusion criteria：

1.Retrospective cohort: pregnant women without complete key information will be excluded.
2.Prospective cohort: participants will be excluded from the prospective cohort if they: withdraw from the study, transfer care to non-participating hospitals, or become lost to follow-up despite multiple contact attempts.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

回顾性队列数据的采集主要依据项目组提供的指引，从病历系统中导出数据，并由项目组统一整理，然后在数据管理系统发布。前瞻性队列设计有专门的信息采集表，电子化后搭载于数据管理系统。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

For the retrospective cohort, data extraction was performed according to project-specific protocols, where raw data were retrieved from medical record systems, underwent centralized curation by the project team, and were subsequently published on the data management system. For the prospective cohort, study-specific electronic case report forms (eCRFs) were designed and deployed on the data management system.
