研究目的：

评价发病24h内机械取栓成功再通后序贯替罗非班治疗的有效性和安全性  

Objectives of Study：

To assess whether sequential administration of tirofiban can enhance the prognosis of neurological function in patients with acute anterior circulation large vessel occlusion within 24 hours of stroke onset, following successful recanalization after mechanical thrombectomy.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄≥18岁； 2.NIHSS评分：6-30分； 3.症状开始到随机时间24h，包括醒后卒中或无目击者卒中患者； 症状开始的时间定义为“ 最后表现正常时间”； 4.此次卒中前mRS 0-1； 5.CTA或MRA证实以下部位急性闭塞导致的临床症状：ICA颅内段、大脑中动脉M1段、大脑中动脉M2主干； 6.平扫CT或DWI的ASPECTS≥6； 7.机械取栓治疗结束后mTICI稳定在2b/3的血流，且其他非责任血管供血流域未出现继发栓塞，或机械取栓前诊断性造影提示闭塞血管改善达到mTICI 2b/3，且不再计划机械取栓操作； 8.受试者或合法代理人能够签署知情同意书.

Inclusion criteria

1.Age >=18 years old; 2.NIHSS score: 6-30; 3.Onset of symptoms to 24h of randomization, including patients with wake-up stroke or unwitnessed stroke; The onset time of symptoms was defined as the last time of normal performance; 4.mRS 0-1 before the stroke; 5.Clinical symptoms caused by acute occlusion of the following sites confirmed by CTA or MRA: intracranial segment of ICA, M1 segment of middle cerebral artery, M2 trunk of middle cerebral artery; 6.ASPECTS>=6 on NCCT or DWI; 7.After the end of mechanical thrombectomy, the mTICI was stable at 2b/3 blood flow, and there was no secondary embolism in the blood supply area of other non-offending vessels, or the diagnostic angiography before mechanical thrombectomy showed that the occluded vessels improved to mTICI 2b/3, and no mechanical thrombectomy was planned; 8.Subjects or a legal representative were able to sign informed consent.

排除标准：

1.动脉内溶栓； 2.血管内治疗前24h内使用过替罗非班； 3.已知妊娠期或哺乳期女性，或随机化前妊娠试验阳性； 4.对替罗非班、放射造影剂或镍钛合金材料过敏; 5.卒中前30天内有任何活动性出血或近期出血（胃肠道、尿路出血等）； 6.卒中前14天内进行实质器官手术、活检术； 7.肝素诱发血小板减少症病史； 8.血小板计数＜100×10^9/L； 9.正在进行血液透析或腹膜透析；已知严重肾功能不全（肾小球滤过率<30ml/min 或血肌酐>220μmol/L）； 10.需要血液透析或腹膜透析的受试者，或出于任何原因有血管造影禁忌证的受试者； 11.预期生存时间小于半年（如合并恶性肿瘤、严重心肺疾病等）； 12.已经参与可能会对结局评估产生影响的其他干预性临床研究； 13.研究者认为不适合参与本研究或者可能会对患者造成显著风险的其他情形（如因精神疾患、认知或情绪障碍无法理解和/或服从研究程序和/或随访）; 14.已出现大脑中线移位或脑疝，脑室占位效应； 15.CT 或 MRI 提示急性颅内出血； 16.新发双侧急性卒中或颅内多流域大血管闭塞； 17.单纯颈内动脉颅外段闭塞。

Exclusion criteria：

1. Intra-arterial thrombolysis; 2. Tirofiban has been used within 24 hours before endovascular treatment; 3. Known pregnant or lactating females, or positive pregnancy test before randomization; 4. Allergy to tirofiban, radiocontrast agent or nitinol material; 5. Any active bleeding or recent bleeding (gastrointestinal, urinary tract bleeding, etc.) within 30 days before stroke; 6. Solid organ surgery and biopsy within 14 days before stroke; 7. History of heparin-induced thrombocytopenia; 8. Platelet count< 100×10^9/L; 9. Undergoing hemodialysis or peritoneal dialysis; Known severe renal insufficiency (glomerular filtration rate <30ml/min or serum creatinine >220μmol/L); 10. Subjects requiring hemodialysis or peritoneal dialysis, or subjects with contraindications to angiography for any reason; 11. The expected survival time is less than half a year (such as malignant tumors, severe cardiopulmonary diseases, etc.); 12. Have participated in other interventional clinical studies that may have an impact on outcome assessment; 13. Other conditions that in the opinion of the investigator are not suitable for participating in this study or may pose significant risks to the patient (such as inability to understand and/or obey the study procedures and/or follow-up due to mental disorders, cognitive or emotional disorders); 14. Midline displacement or herniation of the brain, ventricular mass effect; 15. CT or MRI showing acute intracranial hemorrhage; 16. New bilateral acute stroke or intracranial multi-basin large vessel occlusion; 17. Isolated extracranial segment occlusion of the internal carotid artery.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

本研究采用纸质病例报告表（CRF）和电子数据捕获系统（Electronic Data Capture System，EDC）进行数据采集和录入，系统中所有试验方案要求的内容都必须提供，未填写内容应予以解释，需要在EDC系统每一个表单下备注处填写原因。
