研究目的：

（1）了解老年慢性病共病患者出院后用药依从性随时间变化的轨迹、轨迹类型及预测因素。 （2）探索不同轨迹组的老年慢性病共病患者用药依从性的促进与阻碍因素，总结针对不同轨迹患者用药依从性的提升建议。  

Objectives of Study：

(1) To understand the trajectory, trajectory types and predictive factors of medication adherence in elderly patients with multimorbidity after discharge over time. (2) To explore the promoting and hindering factors of medication adherence in elderly patients with multimorbidity in different trajectory groups, and summarize the suggestions for improving medication adherence in patients with different trajectories.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄≥60 岁； 2.在二级及以上医院确诊患有不少于 2 种慢性病； 3.出院后服用时长在 6 个月以上的处方药物≥1 种； 4.福州市区常住居民，居住时间＞6个月。

Inclusion criteria

1. Age >=60 years old; 2. Be diagnosed with no less than two chronic diseases in a secondary-level or higher hospital; 3. Take at least one prescription drug for a duration of more than 6 months after discharge; 4. Permanent residents of Fuzhou urban area who have resided for more than six months.

排除标准：

1.严重认知障碍或精神疾病，无法理解研究要求； 2.合并严重心、肝 、肾功能不全及恶性肿瘤； 3.已参与其他可能影响本研究结果的临床试验。

Exclusion criteria：

1. Severe cognitive impairment or mental illness, unable to understand the research requirements; 2. Combined with severe heart, liver and kidney insufficiency and malignant tumors; 3. Has participated in other clinical trials that may affect the results of this study.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

由经过专门培训的资料收集员对数据进行测量和采集。采用Excel建立数据库，为保证数据录入的准确性，所有数据均双人录入；采用SPSS软件进行数据处理。
