本研究旨在评估隐形矫治器联合高位头帽J钩对青少年骨性II类错颌畸形的三维颌骨调控效果，重点关注矢状向及垂直向的改善作用。  

Objectives of Study：

This study aims to evaluate the three-dimensional skeletal control effects of clear aligners combined with high-pull headgear J-hook traction in adolescents with skeletal Class II malocclusion, with a focus on sagittal and vertical improvement.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

回顾2018-2023年完成治疗的青少年患者，年龄9-14岁，诊断为骨性II类错颌畸形（ANB角>=5°，Wits值>=3mm），垂直生长型（SN-MP>=35°，FMA>=28°），处于生长发育高峰期（年身高增长>=5cm），且治疗前后影像学数据完整。

Inclusion criteria

The adolescent patients aged 9-14 years who completed treatment from 2018 to 2023 were retrospectively diagnosed with bony class II malocclusion (ANB angle>=5°, Wits value>=3mm), vertical growth type (SN-MP>=35°, FMA>=28°), were at the peak of growth and development (annual height growth >=5cm), and the imaging data before and after treatment were complete.

排除标准：

1.先天性颅面畸形（如唇腭裂）; 2.颞下颌关节紊乱病史; 3.全身性骨代谢疾病; 4.既往正畸治疗史; 5.治疗依从性不足（J钩/肌激动器佩戴时间<10小时/天）.

Exclusion criteria：

1.Congenital craniofacial deformities (e.g., cleft lip and palate); 2.History of temporomandibular joint disorders (TMD); 3.Systemic bone metabolism disorders; 4.Previous orthodontic treatment history; 5.Poor treatment compliance (J-hook activator wear time <10 hours/day).

本研究采用标准化病例记录表（Case Record Form, CRF）与电子数据采集系统（Electronic Data Capture, EDC）协同管理数据。CRF设计严格遵循研究方案，涵盖人口统计学、临床检查、影像学参数等模块，由培训合格的研究员进行双盲录入。EDC系统选用ResMan平台（基于互联网的临床研究管理系统），实现数据实时录入、逻辑校验、版本控制和审计追踪功能。所有数据变更均需电子签名确认，确保符合ALCOA+原则（可归因、清晰、同步、原始、准确、完整、一致、持久、可用）。系统权限分级管理，研究协调员（CRC）负责数据质疑处理，数据管理员（DM）执行定期质量控制（QA）。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

A standardized data collection and management system was implemented, comprising a Case Report Form (CRF) and an Electronic Data Capture (EDC) system. The CRF was designed to align with the study protocol, capturing demographic data, clinical assessments, and imaging parameters. Data were double-entered by trained personnel to ensure accuracy. The EDC system (ResMan, an internet-based platform) enabled real-time data entry, automated logical checks, version control, and audit trails. All data modifications required electronic signatures, adhering to ALCOA+ principles (Attributable, Legible, Contemporaneous, Original, Accurate, Complete, Consistent, Enduring, Available). Role-based access control was enforced, with Clinical Research Coordinators (CRCs) managing data queries and Data Managers (DMs) performing quality assurance (QA).
