研究目的:

通过比较基于神经节细胞的特征性病理改变的分级系统与现有切缘石蜡病理金标准的差异,构建先天性巨结肠术中快速病理检查切缘病理标准,并验证其诊断效能与一致性。  

Objectives of Study:

By comparing a grading system based on characteristic pathological changes of ganglion cells with the current gold standard of paraffin-embedded pathological assessment, we aim to establish intraoperative rapid pathological criteria for surgical margin evaluation in congenital aganglionosis and to validate its diagnostic efficacy and consistency.

Description for medicine or protocol of treatment in detail:

 

纳入标准:

1. 经组织学病理证实为先天性巨结肠的患者,包括短端型、长段型及全结肠型巨结肠; 2. 完整的临床资料及病理信息; 3. 签署参加本研究的书面知情同意书。

Inclusion criteria

1. Patients with histopathologically confirmed Hirchsprung Disease, including short-segment HSCR, long-segment HSCR and total colon aganglionosis. 2. Complete clinical data and pathological information; 3. Patients who signed written informed consent to participate in the study.

排除标准:

1. 最终诊断为先天性肠神经元发育不良者; 2. 最终诊断为慢性假性肠梗阻者; 3. 明确染色体异常及合并其他先天性疾病者。

Exclusion criteria:

1. The pathologic diagnosis is intestinal neuronl dysplasia; 2. The pathologic diagnosis is chronic intestinal pesudo-obstruction. 3. Combined with chromosomal abnormalities and other congenital disorders.

金标准或参考标准(即可准确诊断某疾病的单项方法或多项联合方法,在本研究中用于诊断是否有该病的临床参考标准):

病理金标准:整圈肠管病理切片阅片,存在以下任何一种情况则视为移行段:①整圈肠管超过1/8缺乏神经节细胞;②整圈肠管超过1/8存在肌间及黏膜下神经节细胞减少;③每400倍镜视野下有超过2处神经纤维直径大于40 μm。

Gold Standard or Reference Standard (The clinical reference standards required to establish the presence or absence of the target condition in the tested population in present study):

Pathologic gold standard of transition zone:(1) complete absence of ganglion cells of a segment equal to >1/8 of the circumference in the submucosal and/or myenteric plexuses (2) a segment equal to >1/ 8 of the circumference in myenteric plexuses composed of only sparse small ganglia. (3)crowded large submucosal nerves and/or nerves >40 mm in diameter per 400 field.

Index test:

Based on the intraoperative rapid pathological grading system for congenital Hirschsprung disease used at our institution, the slides were classified into six categories according to the assessments of two senior pathologists.

所有病理切片遮蔽原始编号后,由专人将随机分配的切片号进行标注,所有病理切片均由2名从事小儿胃肠病理学的专家进行单盲独立阅片,不预先知晓各自阅片结果,并由专职人员记录阅片结果,并由额外一名病理科医生单独阅片2次进行质量审核

Blinding:

All pathological slides were anonymized by masking the original identification numbers and relabeled with randomly assigned codes by a designated individual. Each slide was independently reviewed in a single-blinded manner by two experts specializing in pediatric gastrointestinal pathology, with no prior knowledge of each other’s evaluations. The results were recorded by a dedicated staff member, and quality control was performed by an additional pathologist who independently reviewed all slides twice
