研究目的:

本项目拟利用小脑齿状核脑深部电刺激术改善脑卒中后肢体运动功能障碍及脑神经重塑功能  

Objectives of Study:

This project aims to utilize deep brain stimulation (DBS) targeting the cerebellar dentate nucleus to improve post-stroke limb motor dysfunction and enhance neural remodeling capacity.

药物成份或治疗方案详述:

 

Description for medicine or protocol of treatment in detail:

 

纳入标准:

1.年龄18-75岁; 2.12-36个月内明确诊断为脑卒中(脑梗死或脑出血); 3.持续性中度至重度上肢偏瘫,即FMA-UE评分≤42分,且上肢运动能力足以进行康复(即FMA-UE的肘关节屈曲、肘关节伸展或手指屈曲伸展评分≥1); 4.具备标准生理运动通路反应,即在部分收缩的手指总伸肌或近端肌肉(包括前臂或腕屈肌)中,运动诱发电位峰间大小≥100μV; 5.受试者本人或合法代理人能够签署知情同意书

Inclusion criteria

1. Age 18-75 years 2. confirmed diagnosis of stroke (ischemic or hemorrhagic) within 12-36 months 3. persistent moderate to severe upper limb hemiplegia, defined as Fugl-Meyer Assessment of Upper Extremity (FMA-UE) score <=42, with sufficient residual motor function for rehabilitation (i.e., FMA-UE scores >=1 for elbow flexion, elbow extension, or finger flexion/extension) 4. preserved standard physiological motor pathway response, evidenced by motor evoked potential (MEP) peak-to-peak amplitude >=100μV in partially contracted extensor digitorum communis or proximal muscles (including forearm or wrist flexors) 5. ability of the subject or their legal guardian to provide written informed consent.

排除标准:

1.上肢肌肉过度痉挛或挛缩; 2.确诊其他神经精神系统疾病(阿尔兹海默症,肌萎缩侧索硬化,帕金森病等); 3.存在神经外科手术禁忌症等; 4.CT/MRI 扫描禁忌症(如幽闭恐惧症); 5.已知处于妊娠期或哺乳期的女性,或随机化前妊娠试验阳性; 6.存在全身麻醉禁忌症(如严重心律失常、严重贫血、肝肾功能异常等); 7.预期生存期小于12个月.

Exclusion criteria:

1. Severe spasticity or contracture of upper limb muscles 2. diagnosed neurological/psychiatric disorders (e.g., Alzheimer's disease, amyotrophic lateral sclerosis [ALS], Parkinson's disease) 3. contraindications to neurosurgical procedures 4. contraindications to CT/MRI scanning (e.g., claustrophobia) 5. female subjects with known pregnancy, lactation, or positive pregnancy test prior to randomization 6. contraindications to general anesthesia (e.g., severe arrhythmia, severe anemia, abnormal hepatic/renal function) 7. life expectancy <12 months.
