研究目的：

本研究旨在探讨艾司氯胺酮对老年患者腰椎手术后睡眠质量和疼痛的影响，为老年患者腰椎手术的麻醉管理提供新的思路。  

Objectives of Study：

The aim of this study was to investigate the effects of esketamine on sleep quality and pain after lumbar spine surgery in elderly patients, and to provide new ideas for anesthesia management of lumbar spine surgery in elderly patients.

纳入标准：

自愿参加本实验，年龄65-80岁；ASA分级Ｉ-Ⅲ级，计划择期全麻下行腰椎手术的患者

Inclusion criteria

Patients who volunteered to participate in this trial, aged 65-80 years; ASA classification I-III, scheduled for elective lumbar spine surgery under general anesthesia.

排除标准：

非计划或紧急手术；心功能不全或严重肺部疾病（Ｉ型或Ⅱ型呼衰）；严重肝肾功能疾病（Child-PughＣ级、需要肾脏替代治疗）；合并精神疾病、认知功能异常、长期服用抗精神病类药物或吸毒者；对研宄所用药物有过敏或禁忌症者；文盲或不能理解研究中的量表

Exclusion criteria：

Unscheduled or emergency surgery; cardiac insufficiency or severe lung disease (type I or II expiratory failure); severe hepatic or renal dysfunction (Child-Pugh class C, need for renal replacement therapy); comorbid psychiatric disorders, cognitive abnormalities, long-term use of antipsychotics, or drug addiction; allergies or contraindications to medications used in the study; illiteracy or inability to comprehend the scales in the study.

麻醉诱导时静脉推注艾司氯胺酮0.5mg/kg负荷剂量，随后以0.1mg·kg-1·h-1持续泵注至缝皮；离开手术室前连接静脉镇痛泵，镇痛泵配药方案：艾氯胺酮50mg和舒芬太尼1.5ug·kg-1（最高150ug）加生理盐水稀释至100ml

At induction of anesthesia, a loading dose of esketamine 0.5 mg/kg was injected intravenously, followed by continuous pumping at 0.1 mg·kg-1·h-1 to the suture skin; an intravenous analgesic pump was connected before leaving the operating room, and the dosing regimen for the pump was: esketamine 50 mg and sufentanil 1.5 ug·kg-1 (up to 150 ug) diluted to 100 ml with saline
