研究目的：

本研究的总目标是初步揭示时钟基因BMAL1对颈动脉斑块易损性的调控作用，并探索以BMAL1为靶点干预颈动脉斑块易损性的可行性，最终为临床防治颈动脉斑块破裂提供新的理论依据和治疗策略。  

Objectives of Study：

The overall goal of this study is to preliminarily reveal the regulatory effect of the clock gene BMAL1 on the vulnerability of carotid plaques, explore the feasibility of intervening in the vulnerability of carotid plaques with BMAL1 as the target, and ultimately provide new theoretical basis and treatment strategies for the clinical prevention and treatment of carotid plaque rupture.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.脑梗死组入选标准：1）年龄≥18岁；2）入组前7天内发生前循环缺血性脑卒中；3）符合中国急性缺血性脑卒中诊治指南的诊断标准，并经头颅CT或(和)MRI检查证实；4）根据TOAST分型为大动脉粥样硬化型；5）无论狭窄程度如何，一侧颈内动脉至少有一个与卒中症状相符的动脉粥样硬化斑块。
2.稳定组符合以下标准：1）年龄≥18岁；2）既往无缺血性心脑血管病史；3）无论狭窄程度如何，一侧颈内动脉至少有一个动脉粥样硬化斑块。
3.健康对照组为健康成年人。

Inclusion criteria

1.Cerebral infarction group inclusion criteria: 1) age >= 18 years old; 2) occurrence of anterior circulation ischemic stroke within 7 days before enrollment; 3) meet the diagnostic criteria of Chinese guidelines for diagnosis and treatment of acute ischemic stroke, and confirmed by head CT or/and MRI examination; 4) Large artery atherosclerosis according to TOAST classification; 5) presence of at least one atherosclerotic plaque in one ICA consistent with stroke, regardless of the degree of stenosis. 2.The stable group met the following criteria: 1) age >= 18 years; 2) no previous history of ischemic cardiovascular disease; 3) presence of at least one atherosclerotic plaque in one ICA regardless of the degree of stenosis. 3.The healthy control group consisted of healthy adults.

排除标准：

1.排除标准：1）根据TOAST标准存在心栓塞性、腔隙性或异常的卒中病因；2）存在血流动力学卒中；3）既往颈动脉手术或支架植入术；4）存在影响预期寿命小于1年的合并症；5）采血时并发感染的；6）因其他原因无法配合完成血液检验及颈动脉超声检查的。

Exclusion criteria：

1.Exclusion criteria: 1) cardioembolic, lacunar, or abnormal stroke etiology according to the TOAST criteria;
2) presence of hemodynamic stroke;
3) previous carotid artery surgery or stenting;
4) comorbidities affecting life expectancy less than 1 year;
5) complicated with infection during blood collection;
6) Unable to cooperate with the blood test and carotid ultrasound examination due to other reasons.

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

1. Data collection: a unified electronic data collection system (EDC) or paper CRF(case report form) was used, and all data collection tools needed to be approved by the ethics committee. Data collection personnel need to be professionally trained and proficient in the use of data collection tools and data quality control standards. Data collection should follow standardized procedures to ensure data integrity and accuracy. For paper CRFS, waterproof and antifouling materials should be used and stored properly. 2. Management of source data and its documents: All source data (including paper CRF and electronic data) shall be properly kept, and a complete file management system shall be established. Paper CRFS should be stored in a safe, dry, and ventilated environment in numbered order, and the corresponding catalogue and index should be established. Electronic data should be stored on a secure server and backed up regularly. All data files should be versioned and clearly identified as version numbers and dates of modification. Data storage location and access should be strictly controlled and restricted to authorized personnel.
