本研究旨在探讨胸骨拉钩在充气纵隔镜食管癌切除术中的应用效果及其临床价值。具体目标包括：评估胸骨拉钩在手术过程中的操作便利性和安全性，分析其对手术时间、出血量及并发症发生率的影响。通过对比传统手术方法与胸骨拉钩辅助技术在食管癌切除术中的效果，探讨其在提高手术成功率和患者术后恢复方面的优势。本研究的主要终点指标为手术时间，手术时间可量化操作便捷性，且与术中创伤、术后恢复相关。次要终点指标包括术中出血量、术中血压波动、术后并发症发生率：包括吻合口瘘、肺部感染、心律失常等。这些指标将综合评估胸骨拉钩在充气纵隔镜食管癌切除术中的应用效果。结合临床数据，分析胸骨拉钩在不同类型食管癌患者中的适用性，为临床实践提供科学依据。  

Objectives of Study：

This study aims to investigate the application effects and clinical value of the sternum retractor in pneumomediastinoscopic esophagectomy for esophageal cancer.The specific objectives include:assessing the ease of use and safety of the sternum retractor during the surgical procedure,and analyzing its impact on surgical time,blood loss,and complication rates.By comparing the outcomes of traditional surgical methods with those of esophagectomy assisted by the sternum retractor,the study explores the advantages of the retractor in improving surgical success rates and postoperative patient recovery.The primary endpoint of this study is surgical time,which can quantify the ease of operation and is related to intraoperative trauma and postoperative recovery.Secondary endpoints include intraoperative blood loss,intraoperative blood pressure fluctuations,and postoperative complication rates,such as anastomotic leakage,pulmonary infection,and arrhythmias.These indicators will comprehensively evaluate the application effects of the sternum retractor in pneumomediastinoscopic esophagectomy for esophageal cancer.Combined with clinical data,the study will analyze the applicability of the sternum retractor in different types of esophageal cancer patients,providing a scientific basis for clinical practice.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①病理诊断已经确诊为食管癌的患者；②手术方式为充气纵隔镜联合腹腔镜；③胸骨解剖结构正常，无胸骨拉钩使用禁忌症（如胸骨畸形、严重骨质疏松）。④患者知情同意，自愿参加本研究。

Inclusion criteria

1 Patients who have been pathologically diagnosed with esophageal cancer; 2 The surgical approach involves pneumomediastinoscopy combined with laparoscopy; 3 Patients with a normal sternum anatomy and no contraindications for the use of a sternum retractor(such as sternum deformity or severe osteoporosis); 4 Patients who have provided informed consent and voluntarily agree to participate in this study.

排除标准：

存在胸骨拉钩使用禁忌症（如胸骨骨折史、胸骨感染）。

Exclusion criteria：

Contraindications for the use of a sternum retractor exist(such as a history of sternum fracture or sternum infection).

Blinding：

This study employs a single-blind design.Participants are blinded to the group assignment before and during the surgery,meaning they are unaware of whether they have been allocated to the experimental group or the control group.The study implementers(such as the surgeons)are aware of the group assignments but are required to strictly adhere to the study protocol to avoid any suggestions or influences on the participants.All outcome assessors unrelated to the surgery(such as postoperative follow-up physicians and data collectors)remain blinded to the group assignments to minimize measurement bias.

本研究数据采集和管理采用手麻系统病例记录表和医院的电子数据采集与管理系统（HIS）。涵盖人口统计学信息、基线特征、干预措施、随访数据和不良事件等必要数据点，由研究协调员和护士填写，并经资深成员审核。数据通过手麻系统进行电子采集和管理，具备数据验证、自动提醒和数据锁定功能。数据录入后由数据管理员和统计师审核，所有修改均记录并审核，确保可追溯性。研究团队定期备份数据，严格遵守数据保护法规，确保数据安全和保密。数据收集完成后，将进行数据清理，处理缺失数据、异常值和重复数据，随后锁定数据，确保其稳定性和不可更改性。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Data collection and management in this study are conducted using the anesthesia and surgery management system's case record form and the hospital's electronic data capture and management system(HIS).These tools cover essential data points,including demographic information,baseline characteristics,interventions,follow-up data,and adverse events.The forms are filled out by the study coordinator and nurses and reviewed by senior team members.Data are electronically collected and managed through the anesthesia and surgery management system,which features data validation,automatic reminders,and data locking.After data entry,the data administrator and statistician review the data,with all modifications recorded and audited to ensure traceability.The research team regularly backs up data and strictly complies with data protection regulations to ensure data security and confidentiality.After data collection is completed,data cleaning is performed to address missing data,outliers,and duplicate data,followed by data locking to ensure stability and immutability.
