研究目的: |
自身免疫性胰腺炎(AIP)发病率逐年升高。现有综合诊断体系繁杂,包括形态学改变在内的多维度临床信息,缺少特异性生物标志物。超声内镜(EUS)能提供AIP形态学诊断的所有要素,但学习曲线长、观察者差异大,难以普及推广。本项目三地合作单位前期发现,肠道肺炎克雷伯菌(KP)诱导滤泡辅助性T细胞(Tfh)活化是AIP重要机制,但菌株致病成分鉴定和临床验证待探索;我们前期建立了AIP全国多中心队列,已提取EUS声像特征建立评分模型,但仍需智能化辅助提高效能。故我们拟整合肠菌、Tfh 活化标志物和EUS声像特征建立人工智能辅助的AIP多模态诊断体系。本研究将通过多中心合作,①鉴定和验证KP菌体中诱导Tfh活化的成分;②提取EUS胰腺声像特征,优化人工智能辅助诊断算法;③建立并验证基于临床信息、生物标志物和EUS的人工智能辅助多模态诊断体系。为临床提供准确度高、便利且易于推广的AIP诊断和治疗评估方法。 |

Objectives of Study: |
The incidence of autoimmune pancreatitis (AIP) has been increasing year by year. The current comprehensive diagnostic system is complex, involving multidimensional clinical information, including morphological changes, and lacks specific biomarkers. Endoscopic ultrasound (EUS) can provide all the elements for the morphological diagnosis of AIP, but its long learning curve and significant inter-observer variability make it difficult to popularize. Previous research from our three collaborating institutions has identified that Klebsiella pneumoniae (KP)-induced activation of follicular helper T cells (Tfh) is an important mechanism of AIP, though the pathogenic components of the bacterial strains and clinical validation are yet to be explored. We have previously established a national multi-center AIP cohort and extracted EUS morphological features to build a scoring model, but further intelligent assistance is needed to improve its efficiency. Therefore, we plan to integrate gut microbiota, Tfh activation markers, and EUS morphological features to establish an AI-assisted multimodal diagnostic system for AIP. This study, through multi-center collaboration, will: ① identify and validate components in the KP bacterium that induce Tfh activation; ② extract EUS pancreatic morphological features and optimize AI-assisted diagnostic algorithms; ③ establish and validate an AI-assisted multimodal diagnostic system based on clinical information, biomarkers, and EUS. |

1. From the beginning to the end of the study, patients who were suspected or diagnosed with AIP at Peking Union Medical College Hospital and related research centers, not limited by gender. |

1. 一般情况差,存在严重的心肺疾病,难以耐受超声内镜检查; |
