研究目的：

通过本研究，拟筛选出胸腹联合腔镜下食管癌根治术后延迟气管拔管的危险因素，并建立相关预测模型。以便早期发现延迟拔管的潜在危险因素，识别延迟拔管高危患者，判断是否拔管及选择合适的拔管时机；帮助麻醉医生和外科医生加强患者围麻醉期管理，降低延迟拔管的发生率，促进患者更快康复。  

Objectives of Study：

Through this study, we aim to screen out the risk factors for delayed tracheal extubation after thoracoscopic and laparoscopic radical esophagectomy and establish a relevant predictive model. This will facilitate early detection of potential risk factors for delayed extubation, identification of high-risk patients, determination of extubation feasibility, and selection of appropriate extubation timing. It will also help anesthesiologists and surgeons strengthen perioperative management, reduce the incidence of delayed extubation, and promote faster patient recovery.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄18-78岁的患者； 2.美国麻醉医师协会分级（American society of anesthesiologists,ASA）Ⅰ-Ⅲ。

Inclusion criteria

1.Patients aged 18-78 years; 2. American Society of Anesthesiologists (ASA) classification I-III.

排除标准：

1.“高风险”拔管患者（指患者存在术前判断为困难气道、术中气道管理风险增加、术后再插管受限、饱胃、合并一项或多项拔管危险因素，拔管后可能需要再次插管且再次插管困难的情况）; 2.合并重要脏器（心、脑、肺、肾等）严重基础疾病的患者; 3.术后机械通气时长＞48h者； 4.术中大出血(大于500ml); 5.血流动力学不稳定者。

Exclusion criteria：

1.Patients at high risk of extubation (referring to patients with preoperative assessment of difficult airway, increased risk of intraoperative airway management, limited postoperative reintubation options, full stomach, and one or more extubation risk factors, who may require reintubation after extubation and face difficulty in reintubation); 2.Patients with severe underlying diseases in vital organs (heart, brain, lungs, kidneys, etc.); 3.Patients with postoperative mechanical ventilation duration >48 hours; 4.Patients with intraoperative massive hemorrhage (more than 500ml); 5.Patients with hemodynamic instability.
