研究目的：

目前，用于糖尿病足溃疡方面的报道仍然很少，本课题拟对自体脂肪组织微片段移植治疗未愈合糖尿病足溃疡的影响和机制进行探讨。该研究将为糖尿病足溃疡创面的修复提供一种新的治疗方法，具有重要的临床意义。  

Objectives of Study：

At present, there are still few reports on the use of diabetic foot ulcers. This study aims to explore the effect and mechanism of autologous adipose tissue microfragment transplantation in the treatment of unhealed diabetic foot ulcers. This study will provide a new treatment method for the repair of diabetic foot ulcer wounds, which has important clinical significance.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄>=18岁但<=75岁，1型或2型糖尿病； 2.ABI（踝肱指数最大值）0.6-1.3； 3.Wagner 3 或 4 级糖尿病足溃疡； 4. 溃疡面积>=1 cm^2（清创术后尽量去除失活、坏死及纤维组织后面积）。如患者有多处病变，需选择一处病变作为合格认证和后续评估的目标病变； 5.糖尿病足溃疡标准治疗1周创面缩小<=20%者。

Inclusion criteria

1. Age >= 18 years but <= 75 years, with type 1 or type 2 diabetes; 2. ABI (Ankle-Brachial Index maximum value) 0.6-1.3; 3. Wagner stage 3 or 4 diabetic foot ulcers; 4. Ulcer area >= 1 cm^2 (after debridement, aim to remove necrotic, dead, and fibrous tissue as much as possible). If the patient has multiple lesions, one lesion must be selected as the qualifying lesion for certification and subsequent evaluation; 5. Standard treatment for diabetic foot ulcers with a wound area reduction of <= 20% in one week.

排除标准：

1.患者在研究开始前3个月内有明显急性心脑血管疾病发作史，如急性心肌梗塞、严重心绞痛、急性中风或短暂性脑缺血发作； 2. 患有严重不能控制的高血压，筛选时收缩压>= 180 mmHg或舒张压>=110 mmHg； 3.重度肾功能不全患者（肌酐> 200μmol/L或肌酐清除率< 30 ml/min）； 4.所有类型的糖尿病性昏迷； 5.曾患有恶性肿瘤或疑似恶性肿瘤（全身体检、腹部超声检查、胸部X线、血液化学检查等）； 6.溃疡部位患有良性肿瘤的患者； 7.患者患有免疫功能低下的疾病，如HIV。使用会干扰评估的药物：如皮质类固醇和免疫抑制剂； 8.妊娠女性或哺乳期女性患者，或计划在研究期间妊娠的男性或女性患者，且治疗期间不愿意采取避孕措施者； 9.健康状况不允许在局部麻醉下进行吸脂手术的患者； 10.研究者判断不适合参加临床试验者。

Exclusion criteria：

1. Patients had a significant history of acute cardiovascular and cerebrovascular disease, such as acute myocardial infarction, severe angina pectoris, acute stroke, or transient ischemic attack, within 3 months before the study began; 2. Have severe uncontrolled hypertension, systolic blood pressure >= 180 mmHg or diastolic blood pressure >=110 mmHg at screening; 3. Patients with severe renal insufficiency (creatinine > 200μmol/L or creatinine clearance < 30 ml/min); 4. All types of diabetic coma; 5. Have had malignant tumors or suspected malignant tumors (general physical examination, abdominal ultrasound examination, chest X-ray, blood chemistry examination, etc.); 6. Patients with benign tumors at ulcer sites; 7. The patient has an immunocompromised disease, such as HIV. Use of drugs that can interfere with assessment: such as corticosteroids and immunosuppressants; 8. Pregnant women or lactating women, or male or female patients who plan to become pregnant during the study period and do not want to take contraceptive measures during treatment; 9. Patients whose health condition does not permit liposuction under local anesthesia; 10. Patients judged by the investigator to be unsuitable for clinical trials.
