研究目的：

通过基于呼吸模式相关的时序性吞咽神经肌肉电刺激进行的呼吸-吞咽协调训练作用于脑卒中后气管切开患者，探讨呼吸-吞咽协调训练对脑卒中后气管切开患者的吞咽功能、呼吸功能、肺部感染严重程度、气管套管拔管率及整体运动功能的影响。  

Objectives of Study：

The study investigates the effect of respiratory-swallow coordination training, based on breathing pattern-related sequential swallowing neuromuscular electrical stimulation, on post-stroke tracheostomy patients. It explores the impact of this training on swallowing function, respiratory function, severity of pulmonary infection, tracheostomy tube decannulation rate, and overall motor function in these patients.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

(1)符合中国急性缺血性脑卒中诊治指南（2018版）和中国脑出血诊治指南（2019 版）脑梗死或脑出血诊断标准，并经颅脑电子计算机断层扫描（ComputedTomography，CT）/磁共振（Magnetic Resonance Imaging，MRI）确诊为新发脑梗死或脑出血； (2)年龄<=80岁，病程<=6个月，合并吞咽功能障碍，采用鼻饲饮食，且染色试验阳性； (3)已行气管切开术，生命体征平稳，吸氧下SPO2>=90％； (4）神志清楚，无严重的认知障碍，能配合执行一般指令； (5）痰量较多，肺部痰鸣音明显，影像学及病原学检查支持肺部感染诊断，且临床肺部感染评分<=６分； (6）未合并心、肝、肾等系统严重疾病及新发下肢深静脉血栓、严重的骨折、关节脱位等； (7）签署治疗知情同意书。

Inclusion criteria

(1) Meets the diagnostic criteria for acute ischemic stroke or intracerebral hemorrhage according to the Chinese Guidelines for the Diagnosis and Treatment of Acute Ischemic Stroke (2018 edition) and the Chinese Guidelines for the Diagnosis and Treatment of Intracerebral Hemorrhage (2019 edition), and has been confirmed by computed tomography (CT) or magnetic resonance imaging (MRI) as newly developed cerebral infarction or cerebral hemorrhage; (2) Age <= 80 years, duration of illness <= 6 months, with complicating swallowing dysfunction, using nasogastric feeding, and positive staining tests; (3) Has undergone tracheotomy, with stable vital signs, and oxygen saturation (SPO2) >= 90% under supplemental oxygen; (4) Conscious, no severe cognitive impairment, able to cooperate and follow general instructions; (5) Produces a large amount of sputum, with significant lung wheezing sound, imaging and pathogen examination supporting the diagnosis of pulmonary infection, and clinical pulmonary infection score <= 6 points; (6) No severe systemic diseases such as heart, liver, kidney, newly developed deep vein thrombosis in lower limbs, serious fractures, or joint dislocation; (7) Signed informed consent for treatment.

排除标准：

(1)生命体征及病情不稳定者； (2)既往存在其他可能致吞咽障碍的疾病如帕金森病、多发性硬化等，或鼻咽癌、头颈部癌、口腔癌等或既往有脑卒中病史且遗留吞咽功能障碍者； (3)肺部感染恶化，临床肺部感染评分＞６分； (4)合并心、肝、肾等系统严重疾病及新发下肢深静脉血栓、严重的骨折、关节脱位等； (5)严重贫血，血红蛋白<=60g/L； (6)治疗不配合者，拒绝签署治疗知情同意书者。

Exclusion criteria：

(1) Patients with unstable vital signs and condition; (2) Those with a history of other diseases that may cause swallowing difficulties, such as Parkinson's disease, multiple sclerosis, etc., or those with a history of nasopharyngeal cancer, head and neck cancer, oral cancer, or previous stroke history with residual swallowing dysfunction; (3) Deterioration of pulmonary infection, with a clinical pulmonary infection score > 6; (4) Severe concurrent systemic diseases of the heart, liver, kidney, etc., newly developed deep vein thrombosis in the lower limbs, serious fractures, dislocations, etc.; (5) Severe anemia, with hemoglobin <= 60 g/L; (6) Non-compliant patients who refuse to sign the informed consent for treatment.
