开展随机对照试验，验证心血管疾病家庭医院整合照护方案的可行性和有效性。  

Objectives of Study：

Conduct randomized controlled trials to verify the feasibility and effectiveness of integrated care programs for cardiovascular diseases in home healthcare.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

① 由专科医生诊断为心血管疾病（包括冠脉疾病、 瓣膜疾病、 心律失常）； ② 患者病情稳定，二级护理； ③ 根据出院/转科标准，由心脏重症监护病房（CCU）转出，至普通病房，或患者意愿直接出院回家； ④ 年龄≥18 岁； ⑤ 愿意持续来院门诊就诊； ⑥ 与主要照护者同住； ⑦ 知情同意并签署同意书。

Inclusion criteria

1.Diagnosed with a cardiovascular condition—such as coronary artery disease, valvular heart disease, or arrhythmia—by a qualified specialist. 2.The patient's clinical condition is stable and currently managed at the secondary care level. 3.The patient has either: been transferred from the Cardiac Intensive Care Unit (CCU) to a general ward in accordance with established discharge/transfer criteria, or been discharged directly to their home. 4.Aged 18 years or older. 5.Willing to attend follow-up visits at the hospital outpatient clinic. 6.Resides with a primary caregiver. 7.Has provided informed consent and signed the consent form.

排除标准：

①患有严重精神疾病或智力受损无法自主表达意愿；②已参与其他健康干预项目。

Exclusion criteria：

1.Diagnosed with a severe mental illness or intellectual disability that impairs the ability to express their own wishes independently. 2.Currently participating in another health intervention program.

Eligible participants will be randomly assigned in a 1:1 ratio to either the intervention group (Hospital-at-Home model) or the control group (standard in-hospital care). Randomization will be conducted using a computer-generated random number sequence to ensure allocation concealment and minimize selection bias.

Blinding：

A single-blind design will be employed. Due to the nature of the intervention, blinding of participants and intervention providers is not feasible. However, outcome assessors, including those responsible for data analysis, will remain blinded to group allocation in order to reduce observer and assessment bias. All outcome measures will be evaluated according to standardized protocols to ensure objectivity and scientific rigor.
