研究目的：

从医患共同决策的理论基础出发，采用课题组前期基于循证和IPDA4.0构建的年龄相关性黄斑变性患者营养决策辅助工具对年龄相关性黄斑变性患者进行干预。探索其应用效果，从而为构建本土化的年龄相关性黄斑变性患者营养决策辅助方案提供科学依据。  

Objectives of Study：

Based on the theoretical basis of doctor-patient joint decision making, the nutrition decision aid tool for age-related macular degeneration patients constructed by the research group based on evidence and IPDA4.0 was used to intervene age-related macular degeneration patients.To explore its application effect, so as to provide a scientific basis for constructing a localized nutrition decision assistance scheme for age-related macular degeneration patients.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

主治医生确诊有年龄相关性黄斑变性疾病；年龄≥45岁；小学及以上文化程度；意识清楚，语言沟通无障碍；了解研究目的，患者及家属知情并同意参与调查。

Inclusion criteria

The attending physician confirmed age-related macular degeneration;Age ≥45 years;Primary school education or above;Clear consciousness and barrier-free language communication;To understand the purpose of the study, patients and their families were informed and agreed to participate in the investigation.

排除标准：

有其他影响视力的眼病或手术病史；严重认知障碍或精神疾病；合并其他全身严重疾病如恶性肿瘤等；沟通无效者。

Exclusion criteria：

Having a history of other eye diseases or operations that affect vision;Severe cognitive impairment or mental illness;Other serious systemic diseases such as malignant tumors;Ineffective communication.

由研究者选取年龄相关性黄斑变性患者106例，根据随机数字表分为试验组和对照组，试验组采用共同决策模式，对照组接受传统决策模式，予方案实施前和实施后，利用《MAPPIN’SDM共同决策量表——观察员部分》《决策困境量表》、《决策准备量表》等方面评价该项目的有效性和可行性。按照上述构建的营养决策辅助工具的方案和技术，采用随机、对照试验设计，陆军军医大学第一附属医院招募受试者。试验组采用年龄相关性黄斑变性患者营养决策辅助工具进行营养决策干预，对照组采用常规的营养健康宣教材料进行干预。

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

根据病例记录表，选择眼科门诊随访年龄相关性黄斑变性患者106例，试验组和对照组各53例，运用SPSS21.0软件对数据进行管理和统计分析
