研究目的：

本研究拟对患者进行术前及术后的颊针干预，评估其对TKA后疼痛的预防及治疗效果，并检测颊针干预对人外周血中AEA、2-AG、AA、前列腺素浓度水平的影响，揭示颊针镇痛效果与内源性大麻素系统及炎症介质之间的潜在联系，为后续颊针镇痛机制的研究提供实践和理论依据。  

Objectives of Study：

This study aims to administer buccal acupuncture interventions to patients both preoperatively and postoperatively, evaluating its preventive and therapeutic effects on pain following total knee arthroplasty (TKA). Additionally, it will examine the impact of buccal acupuncture on the concentrations of AEA, 2-AG, AA, and prostaglandins in human peripheral blood. The study seeks to reveal the potential relationship between the analgesic effects of buccal acupuncture and the endocannabinoid system, as well as inflammatory mediators. The findings will provide both practical and theoretical foundations for further research into the mechanisms underlying buccal acupuncture-induced analgesia.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1）年龄≥18岁，性别、民族不限； 2）ASA分级Ⅰ-Ⅲ级； 3）择期行初次全膝关节置换手术者； 4）能配合理解、完成NRS评估量表； 5）对本研究内容及意义知情，并签署知情同意书；

Inclusion criteria

1) Age >=18 years old, regardless of gender or ethnicity; 2) ASA physical status Ⅰ-Ⅲ; 3) patients undergoing elective primary total knee arthroplasty; 4) able to understand and complete the NRS; 5) informed about the content and significance of this study and signed informed consent;

排除标准：

1)拒绝参加本研究者; 2）颊针穿刺部位有感染、破损者; 3）凝血功能异常者； 4）术前因严重痴呆、语言障碍等无法交流者或不能配合完成NRS疼痛评估量表者; 5)主治医生或研究者认为存在其他不宜参加本研究的情况{如：其他原因引起的疼痛(包括感染、恶性肿瘤等)，及既往已经存在并持续至今的与手术疾病无关的疼痛}；

Exclusion criteria：

1) Refusal to participate in the study; 2) the buccal needle puncture site was infected or damaged; 3) abnormal coagulation function; 4) patients who were unable to communicate before surgery due to severe dementia, language disorder, or unable to cooperate to complete the NRS pain assessment scale; 5) Other conditions considered by the attending doctor or investigator to be unsuitable for the study {e.g., pain caused by other reasons (including infection, malignant tumor, etc.) and pain unrelated to surgical diseases that have existed in the past and continue till now};
