研究目的：

1) 评估双侧颈深淋巴静脉吻合术治疗中、重度AD的疗效和安全性 2) 探索双侧颈深淋巴静脉吻合术治疗中、重度AD的潜在机制 3) 提供支持双侧颈深淋巴静脉吻合术治疗中、重度AD临床实践中应用的循证医学证据  

Objectives of Study：

1) Evaluate the efficacy and safety of bilateral deep cervical lymphaticovenous anastomosis for the treatment of moderate to severe Alzheimer's Disease. 2)Explore the potential mechanisms of bilateral deep cervical lymphaticovenous anastomosis in the treatment of moderate to severe Alzheimer's Disease. 3)Provide evidence-based medical evidence supporting the application of bilateral deep cervical lymphaticovenous anastomosis in clinical practice for the treatment of moderate to severe Alzheimer's Disease.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1）符合美国国家衰老研究所和阿尔茨海默病协会（NIA-AA）发布的《阿尔茨海默病诊断和分期的修订标准（2024年）》，根据生物标志物和临床症状诊断为AD的患者，患者临床分期处于第四阶段至第六阶段。 2）年龄在50岁(含)—90岁(含)之间 3）术前麻醉评估ASA分级可以耐受手术 4）能配合完成神经心理测评、体格检查、颅脑MRI、PET/CT、腰椎穿刺等医学检查 5）自愿参与本研究，提供生物样本，签署知情同意书

Inclusion criteria

1) In accordance with the revised criteria for the diagnosis and staging of Alzheimer's Disease (AD) published by the National Institute on Aging and the Alzheimer's Association (NIA-AA) in 2024, patients diagnosed with AD based on biomarkers and clinical symptoms, with patients' clinical staging in the fourth to sixth stages. 2) Age between 50 years (inclusive) and 90 years (inclusive). 3) Preoperative anesthesia assessment with an ASA classification that can tolerate surgery. 4) Capable of cooperating to complete neuropsychological assessments, physical examinations, cranial MRI, PET/CT, lumbar puncture, and other medical examinations. 5) Voluntarily participate in this study, provide biological samples, and sign the informed consent form.

排除标准：

1）其他原因所致的痴呆：如血管性痴呆、因艾滋病和梅毒等引起的中枢神经系统感染、克-雅氏病、亨廷顿舞蹈症、帕金森病、路易体痴呆、脑外伤性痴呆、药物中毒和酒精中毒等理化因素、肝性脑病和肺性脑病等重要的躯体疾病、甲状腺疾病和甲状旁腺疾病等内分泌系统病变、以及维生素缺乏或其他任何原因引起的痴呆 2）存在先天性智力障碍及严重神经、精神疾病 3）存在严重基础疾病，如心、肝、肾、呼吸功能衰竭或合并全身恶性肿瘤等，预计生存期不足一年者 4）存在手术禁忌症，如凝血功能障碍，对手术相关药物、器械、造影剂过敏，颈部有感染，如皮肤炎症、深部组织感染等 5）家属拒绝手术治疗的患者 6）无法完全配合随访 7）参与其他可能影响本试验的临床试验者。 8）研究者或者发起者认为其他不合适入组的原因

Exclusion criteria：

1) Dementia caused by other reasons: such as vascular dementia, central nervous system infections caused by AIDS and syphilis, Creutzfeldt-Jakob disease, Huntington's chorea, Parkinson's disease, Lewy body dementia, traumatic brain injury dementia, drug and alcohol poisoning and other physicochemical factors, hepatic encephalopathy and pulmonary encephalopathy and other significant somatic diseases, endocrine system disorders such as thyroid and parathyroid diseases, and dementia caused by vitamin deficiencies or any other reasons 2) Presence of congenital intellectual disabilities and severe neurological or psychiatric diseases. 3) Presence of severe underlying diseases, such as heart, liver, kidney, respiratory failure, or combined with systemic malignant tumors, etc., with an expected survival of less than one year 4) Presence of contraindications to surgery, such as coagulation disorders, allergies to surgery-related drugs, instruments, contrast agents, infections in the neck, such as skin inflammation, deep tissue infections, etc 5) Patients whose families refuse surgical treatment 6) Inability to fully cooperate with follow-up 7) Participants in other clinical trials that may affect this trial 8) Other reasons deemed inappropriate for enrollment by the investigator or sponsor
