研究目的：

探究围术期个体化肺开放通气策略对老年患者前列腺癌根治术术后肺不张的影响。  

Objectives of Study：

To explore the effect of perioperative individualized lung open ventilation strategy on atelectasis in elderly patients after radical prostatectomy.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.择期行前列腺癌根治术的老年患者，年龄≥65岁； 2.BMI 18.5～25 kg/m^2； 3.ASA Ⅱ～Ⅲ级； 4.肺部并发症中高风险（ARISCAT 评分≥26）； 5.术前肺功能并未显示明显异常，无脊椎或胸廓畸形，无困难气道、哮喘或慢阻肺； 6.预计手术时间≥2h； 7.自愿受试并签署知情同意书。

Inclusion criteria

1.Patients aged 65 years or older undergoing radical prostatic cancer surgery; 2.BMI 18.5~25 kg/m^2; 3.ASA II~III; 4.Moderate and high risk of pulmonary complications (ARISCAT score>=26); 5.Preoperative pulmonary function showed no significant abnormalities, no spinal or thoracic malformations, no difficult airway, asthma, or COPD; 6.The operation time is expected to be >=2h; 7.Volunteer and sign informed consent.

排除标准：

1.术前高血压未得到有效控制或控制不佳(收缩压≥180mmHg 或舒张压 ≥110mmHg)； 2.严重的呼吸系统合并症(包括但不局限于慢性阻塞性肺疾病、哮喘、肺癌、肺结核等)； 3.严重的肝肾功能损害； 4.严重的心血管系统疾病：如合并严重的心律失常，心力衰竭，急性冠脉综合征等基础疾病； 5.最近3个月参与其他临床研究； 6.其他研究者认为不适合参与研究的情况.

Exclusion criteria：

1.Preoperative hypertension was not effectively controlled or poorly controlled (systolic>=180mmHg or diastolic >=110mmHg); 2.Severe respiratory comorbidities (including but not limited to chronic obstructive pulmonary disease, asthma, lung cancer, tuberculosis, etc.); 3.Severe impairment of liver and kidney function; 4.Serious cardiovascular system diseases: such as severe arrhythmia, heart failure, acute coronary syndrome and other underlying diseases; 5.Participated in other clinical studies in the last 3 months; 6.Situations that other researchers found inappropriate to participate in the study.
