研究目的:

我们拟入组经神经外科医生评估后,考虑行EDAS手术,且患者有意愿行EDAS手术干预的18-65岁烟雾病患者,通过注意网络范式评估术前、术后注意功能,采用核磁Pcasl序列评估EDAS术前及术后5天 血流重建情况,采用脑电、DTI及静息态扫描评估术前、术后功能及结构连接变化,探讨行为与血流灌注的关系,验证假设一:EDAS术后早期出现认知功能改善, EDAS术后早期血流重建是其认知功能改善的生理基础;探讨执行控制网络功能连接与血流重建的关系,验证假设二:EDAS术后血流重建可增强以额叶为中心的执行控制网络连接,从而改善其认知功能。  

Objectives of Study:

We plan to enroll patients aged 18–65 years with Moyamoya disease who are evaluated by neurosurgeons and considered candidates for encephaloduroarteriosynangiosis (EDAS) surgery, and who voluntarily consent to undergo EDAS intervention. Using the Attention Network Task, we will assess pre- and post-EDAS attentional function. Pseudo-continuous arterial spin labeling (pCASL) MRI will be employed to evaluate cerebral blood flow reconstruction at baseline and 5 days post-EDAS. Electroencephalography (EEG), diffusion tensor imaging (DTI), and resting-state functional MRI (rs-fMRI) will be utilized to analyze pre- and post-EDAS changes in functional and structural connectivity. The study aims to: Explore the relationship between behavioral performance and hemodynamic perfusion, testing Hypothesis 1: Early cognitive improvement post-EDAS is physiologically grounded in rapid cerebral blood flow reconstruction. Investigate the association between blood flow reconstruction and functional connectivity within the executive control network, testing Hypothesis 2: EDAS-induced hemodynamic enhancement strengthens frontoparietal executive control network connectivity, thereby driving cognitive recovery.

纳入标准:

1)年龄18-65岁; (2)经神经外科医生评估后,考虑行EDAS手术,且患者有意愿行EDAS手术干预的烟雾病患者 (3)Fazekas分级≤2

Inclusion criteria

(i) aged 18-65 years; (ii) patients diagnosed with moyamoya disease who are considering EDAS surgery after evaluation by two neurosurgeons have expressed intent to proceed with EDAS surgery; (iii) Fazekas scores <= 2.

排除标准:

(1) 脑梗死、脑出血等器质性神经系统疾病病史 (2) 精神疾病病史 (3) 智力低于正常水平或严重认知障碍不能配合认知测评完成 (4) 系统性免疫疾病 (5) 头部外伤、物质或酒精滥用史 (6) 严重躯体疾病 (7) 怀孕 (8) 磁共振扫描禁忌症

Exclusion criteria:

(i) Neurological disorders, except TIA; (ii) Psychiatric illnesses; (iii) Below-normal intelligence or severe cognitive impairment preventing cognitive assessment; (iv) Systemic immune diseases; (v) History of head trauma, substance abuse, or alcohol abuse; (vi) Serious physical illnesses; (vii) Pregnancy; (viii) Contraindications for magnetic resonance scanning

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture:

Data Collection and Management: Research data were collected using standardized Case Record Forms (CRFs) and managed through the ResMan Electronic Data Capture (EDC) system with quality control measures. The CRF encompassed baseline characteristics, surgical parameters, and follow-up metrics, which were entered into the EDC system via dual independent entry by trained researchers. The system incorporated built-in logic validation and version control functionalities, and critical data points underwent verification by independent monitors. Anonymized datasets were stored on encrypted servers with access restricted to authorized personnel only.
