研究目的：

（1）多西环素暴露后预防对预防MSM人群梅毒和生殖道衣原体感染的效果。 （2）了解现阶段在我国MSM人群中开展依托疾控模式的性病化学预防的可行性； （3）了解多西环素暴露后预防对淋球菌和沙眼衣原体耐药的影响。  

Objectives of Study：

1.Effect of post-doxycycline prophylaxis on the prevention of syphilis and genital chlamydia infection in MSM population. 2.To understand the feasibility of chemoprevention of STD based on disease control model in MSM population in China; 3.To understand the effect of doxycycline post-exposure prophylaxis on drug resistance of gonorrhoeae and chlamydia trachomatis.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①年龄大于18周岁及小于70周岁； ②目前未感染梅毒、HIV或生殖道沙眼衣原体； ③出生时性别为男性并有男男性行为经历者，且在入选前6个月内有以下一种或多种危险性行为者: a.发生过无保护同性性行为； b.入选前6个月内的同性性伴总数在2 个或以上； c.自报有除艾滋病外其他性病感染经历，如梅毒、生殖器疱疹、淋病、衣原体感染，软下疳，或性病性淋巴肉芽肿等，但已获得治疗； d.曾经有HIV暴露前或暴露后预防用药经历，或正在参加者； ④近3个月未有四环素类药物服用史； ⑤既往检查无严重的肝肾功能障碍，且没有感染乙肝和丙肝； ⑥全面了解试验目的、设计、可能的结局事件后，自愿参加研究，并愿意签署知情 同意书者。

Inclusion criteria

1.Aged between 18 and 70 years. 2.Currently free from syphilis, HIV, or genital Chlamydia trachomatis infection. 3.Individuals assigned male at birth (AMAB) with a history of male-to-male sexual behavior (MSM), who have engaged in one or more of the following high-risk sexual practices within the past 6 months: (1)Engaged in unprotected anal intercourse (2)Had >=2 male sexual partners (3)Self-reported history of sexually transmitted infections (STIs) other than HIV (e.g., syphilis, genital herpes, gonorrhea, chlamydial infection, chancroid, or lymphogranuloma venereum) with documented treatment completion (4)Previous or current use of HIV pre-exposure prophylaxis (PrEP) or post-exposure prophylaxis (PEP), or ongoing participation in HIV prevention trials 4.No history of tetracycline antibiotic use within the preceding 3 months. 5.Absence of severe hepatic or renal dysfunction based on medical history, with no evidence of active hepatitis B or C virus infections. 6.Demonstrated understanding of study objectives, protocol design, and potential outcomes, with voluntary consent to participate through written informed consent.

排除标准：

①严重过敏体质或对四环素类成分过敏者。 ②严重慢性病，代谢性疾病（如糖尿病等），神经精神障碍患者。 ③生病或者依从性差。

Exclusion criteria：

1.Severe allergies or allergies to tetracycline components. 2.Patients with severe chronic diseases, metabolic diseases (such as diabetes, etc.), neuropsychiatric disorders. 3.Sick or poor compliance.

发生高危行为后24小时内服用200mg多西环素。每天累计服用药物达到200mg则当天不再服用药物。每周累计服用剂量达到600mg后则本周不再服用药物。每三个月进行一次梅毒/HIV检测。

Take 200mg of doxycycline within 24 hours of high-risk behavior. If the cumulative dose reaches 200mg per day, the drug will not be taken on the same day. After the cumulative weekly dose reaches 600mg, the drug will not be taken this week. Syphilis /HIV testing every three months.
