主要目的:1.?评估诊断准确性: 确定超声测量视神经鞘直径(ONSD)和低场强磁共振成像在诊断脑积水方面的准确性和可靠性,与现有诊断方法进行比较分析。 2.?探索联合诊断价值: 研究超声测量ONSD与低场强MRI联合使用时的诊断效能,是否能够提供比单一方法更全面的诊断信息。 3.?监测病程进展: 评估超声测量ONSD在监测脑积水患者病程进展和治疗效果方面的应用价值。 4.?预测患者预后: 探索超声测量ONSD和低场强MRI在预测脑积水患者预后方面的潜力,包括病情恶化和康复的可能性。 5.建立新的诊断标准:探索超声视神经鞘直径在诊断正压性脑积水上的应用的准确性和可靠性 6.初步研究人工智能与临床诊断的有机结合  

Objectives of Study:

Main objectives: 1. To evaluate diagnostic accuracy: To determine the accuracy and reliability of ultrasonic measurement of optic nerve sheath diameter (ONSD) and low field intensity magnetic resonance imaging in the diagnosis of hydrocephalus, and to compare with existing diagnostic methods. 2. Explore the value of combined diagnosis: Study the diagnostic efficiency of ultrasound measurement ONSD combined with low field intensity MRI, and whether it can provide more comprehensive diagnostic information than a single method. 3. Monitoring the progression of disease: To evaluate the application value of ultrasonic measurement of ONSD in monitoring the progression of disease and treatment effect in patients with hydrocephalus. 4. Predicting patient outcomes: Explore the potential of ultrasound measurements of ONSD and low-field MRI in predicting outcomes in patients with hydrocephalus, including the likelihood of disease deterioration and recovery. 5. Establish new diagnostic criteria: Explore the accuracy and reliability of the application of ultrasonic optic nerve sheath diameter in the diagnosis of positive pressure hydrocephalus 6. Preliminary research on the organic combination of artificial intelligence and clinical diagnosis.

纳入标准:

1.大于18周岁的患者,性别不限;
2.存在颅脑外伤、颅内肿瘤、脑炎或脑血管疾病、脑积水患者等,需进行腰椎穿刺术测量脑脊液压力;
3.签署患者知情同意书,并同意按照研究方案的要求参加所有的检查和治疗;

Inclusion criteria

1.Patients older than 18 years old, without gender limitation;
2.Patients with craniocerebral trauma, intracranial tumor, encephalitis or cerebrovascular disease, hydrocephalus, etc., and lumbar puncture is required to measure the cerebrospinal fluid pressure;
3.Signed patient informed consent and agreed to participate in all examinations and treatment as required by the study protocol;

排除标准:

1.已知的眼部病理或视神经损伤史。
2.有腰椎穿刺术禁忌证者;
3.近期有使用影响颅内压药物的患者;
4.研究者认为不符合参与此研究;

Exclusion criteria:

1.Known history of ocular pathology or optic nerve injury;
2.Patients with contraindications for lumbar puncture surgery;
3.Patients with recent use of drugs affecting intracranial pressure;
4.The investigator did not eligible for participation in the study;

Gold Standard or Reference Standard (The clinical reference standards required to establish the presence or absence of the target condition in the tested population in present study):

Clinical symptoms and signs: The patient was observed for symptoms of cranial enlargement and increased intracranial pressure. Head imaging examination showed that the ventricle system was enlarged, and Evans index >0.33 was the landmark index for the diagnosis of hydrocephalus.

Ultrasound optic sheath diameter combined with low-field MRI can assess whether patients have positive pressure hydrocephalus or positive pressure hydrocephalus

目标人群(可以是某种疾病患者或正常人群,详细描述其疾病特征,注意应纳入符合分布特点的全序列病例,具有良好的代表性)

采用病例对照研究设计,选择确诊的脑积水患者作为病例组,选择年龄、性别匹配且无神经系统疾病的健康个体作为对照组。其中病例组又分为两组,一组为正压性脑积水,一组为脑脊液高压脑积水。

A case-control study design was adopted, and patients with confirmed hydrocephalus were selected as the case group, and healthy individuals with age, sex, and no neurological diseases were selected as the control group. The case group was divided into two groups, one was positive pressure hydrocephalus and the other was cerebrospinal fluid hypertensive hydrocephalus.

Data collection using standardized ultrasound equipment and MRI devices to ensure accuracy and reproducibility of the data. Data measurement is a key link to ensure study accuracy and reliability. In diagnostic studies of hydrocephalus, data measurements include but are not limited to the following aspects: Measurement of the ultrasound optic nerve sheath diameter Measurement of ONSD, ETD and OND: using a portable ultrasound instrument (Sonoxacin 9000), the output power is reduced to 5-10 MHz.
