研究目的：

对比使用食道测压和AOP滴定ARDS患者PEEP的两种方法，探索对ARDS患者更有益的PEEP滴定策略  

Objectives of Study：

Compare the two methods of titrating positive end - expiratory pressure (PEEP) in ARDS patients using esophageal pressure and airway opening pressure, and explore a more beneficial PEEP titration strategy for ARDS patients.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄>=18 岁； 2.签署知情同意书； 3.符合 ARDS 的柏林定义标准； 4.患者接受插管和机械通气。

Inclusion criteria

1. Age>=18 years; 2. Sign the informed consent form; 3. Meets the Berlin definition of ARDS; 4. The patient receives intubation and mechanical ventilation.

排除标准：

1.妊娠和哺乳期妇女； 2.有食管导管插入禁忌证； 3.有 EIT 监测禁忌症； 4.患有严重慢性阻塞性肺病（GOLD III-IV 级）； 5.有严重或无法控制的哮喘病史； 6.有肺转移史； 7.正在接受肾脏替代治疗； 8.患有充血性心力衰竭（NYHA 分级 III-IV）。

Exclusion criteria：

1. Pregnant and lactating women; 2. There are contraindications to esophageal catheter insertion; 3. Have contraindications to EIT monitoring; 4. Have severe chronic obstructive pulmonary disease (GOLD III-IV); 5. Have a history of severe or uncontrolled asthma; 6. Have a history of lung metastases; 7. Receiving renal replacement therapy; 8. Have congestive heart failure (NYHA class III-IV).
