研究目的：

通过一次性胃部细胞无创采样器采取患者胃液，并对其中的microRNA、蛋白质等生物标志物进行组学测序研究，筛选出早期胃癌差异microRNA及差异蛋白，以期通过无创方法探索及鉴别新型胃癌生物标志物。本研究旨在可通过无创方法，提高早期胃癌的诊出率，提高患者的生存率，改善生活质量，降低胃癌的疾病负担。  

Objectives of Study：

Using a disposable non-invasive gastric mucosal cell sampler to collect gastric fluid from patients, we conducted omics sequencing studies on biomarkers such as microRNA and proteins. This process aims to identify differentially expressed microRNAs and proteins associated with early-stage gastric cancer. We hope to explore and identify novel biomarkers for gastric cancer through non-invasive methods. This study aims to improve the detection rate of early-stage gastric cancer, thereby enhancing patient survival rates, improving quality of life, and reducing the disease burden of gastric cancer.

纳入标准：

总纳入标准：年龄18岁- 60岁，自愿参与研究并签署知情同意书。 分组纳入标准： （1）胃癌组：根据TNM分期系统（AJCC/UICC）对胃癌进行临床病理分期，早期胃癌包括重度异型增生、原位癌、I期、部分II期，晚期胃癌主要包括III期、IV期，其中晚期胃癌进一步根据Borrmann分型进行镜下分型，分别分为I型（隆起型）、Borrmann II型（溃疡型）、Borrmann III型（浸润型）、Borrmann IV型（弥漫浸润型）。 （2）胃溃疡组：根据AHS（Artificial Helicobacter pylori Staging）分类，将胃溃疡主要分为活动期（Active stage, A）、愈合期（Healing stage, H）、瘢痕期（Scar stage, S）。 （3）萎缩性胃炎组：根据新悉尼系统分类方法，将受试者纳入萎缩性胃炎组，并根据OLGA（operative link for gastritis assessment）评估萎缩范围和严重程度，将萎缩性胃炎分为 I-IV期。 （4）肠化生组：根据病理活检，将明确诊断为肠化生的受试者纳入本组，肠化生GIM分级，主要通过OLGIM （operative link on gastric intestinal metaplasia assessment）评分，将肠化生分为I-IV期。 （5）健康对照组：来我院进行内镜体检的健康志愿者。

Inclusion criteria

General Inclusion Criteria: Age between 18 and 60 years old, with voluntary participation in the study and signed informed consent. Group-specific Inclusion Criteria: (1)Gastric Cancer Group: Gastric cancer was clinically and pathologically staged according to the TNM staging system (AJCC/UICC). Early-stage gastric cancer includes severe dysplasia, carcinoma in situ, Stage I, and some Stage II cases. Advanced gastric cancer mainly includes Stage III and IV cases. Advanced gastric cancer was further classified endoscopically according to the Borrmann classification into Type I (polypoid), Borrmann II (ulcerative), Borrmann III (infiltrative), and Borrmann IV (diffuse infiltrative) . (2)Gastric Ulcer Group: Gastric ulcers were classified according to the Artificial Helicobacter pylori Staging (AHS) system into active stage (A), healing stage (H), and scar stage (S) . (3)Atrophic Gastritis Group: Participants were included in the atrophic gastritis group according to the updated Sydney System classification. The extent and severity of atrophy were assessed using the Operative Link for Gastritis Assessment (OLGA), with atrophic gastritis classified into Stages I–IV . (4)Intestinal Metaplasia Group: Participants with a confirmed diagnosis of intestinal metaplasia were included. The grading of intestinal metaplasia (GIM) was assessed using the Operative Link on Gastric Intestinal Metaplasia Assessment (OLGIM) scoring system, which classifies intestinal metaplasia into Stages I–IV . (5)Healthy Control Group: Healthy volunteers who underwent endoscopic screening at our hospital.

排除标准：

（1）有全胃切除术或部分胃切除术史； （2）合并有其他胃肠道疾病（如炎症性肠病、其他类型的胃炎等）； （3）处于妊娠期或哺乳期的妇女； （4）在接受胃镜检查前1个月内，有抗生素、PPI等口服药物治疗史； （5）处于急性上消化道出血、穿孔期； （6）合并有心、脑、肺、肝、肾等严重脏器等疾病。

Exclusion criteria：

(1)A history of total or partial gastrectomy. (2)Presence of other gastrointestinal diseases (e.g., inflammatory bowel disease, other types of gastritis, etc.). (3)Women who are pregnant or breastfeeding. (4)History of oral medication use, including antibiotics or proton pump inhibitors (PPIs), within one month prior to undergoing endoscopy. (5)Presence of acute upper gastrointestinal bleeding or perforation. (6)Presence of severe comorbid conditions involving the heart, brain, lungs, liver, kidneys, or other major organs.
