研究目的：

1.主要目的：评估ESD术后淋巴结转移高风险的EGC患者，序贯腹腔镜区域性淋巴结清扫的短期和长期治疗效果，包括局部复发率（包括原位复发和异位复发）、淋巴结或远处转移率、总生存期（OS）、疾病特异性生存期（DSS）和无病生存期（DFS）（3年、5年生存期）。 2.次要目的：并发症（术中、术后7天内、30天内）、住院时长、生存质量（采用标准化生存质量量表进行评估）、治疗费用。  

Objectives of Study：

1.objective: To evaluate the short-term and long-term therapeutic effects of sequential laparoscopic regional lymph node dissection in EGC patients at high risk of lymph node metastasis after ESD, including local recurrence rates (both in situ and ectopic recurrence), lymph node or distant metastasis rates, overall survival (OS), disease-specific survival (DSS), and disease-free survival (DFS) (3-year and 5-year survival rates). 2.Secondary objectives: Complications (intraoperative, within 7 days postoperative, and within 30 days postoperative), length of hospital stay, quality of life (assessed using standardized quality of life scales), and treatment costs.

纳入标准：

参考最新2024年美国NCCN胃癌指南，内镜治疗后经病理评估考虑淋巴结转移高风险的患者，满足以下其中一条: （1）肿瘤组织学类型为低分化； （2）术后病理证实深浸润：pT1b2(SM>500um) ； （3）术后病理证实淋巴脉管浸润。

Inclusion criteria

According to the latest 2024 NCCN guidelines for gastric cancer in the United States, patients at high risk for lymph node metastasis after endoscopic treatment, based on pathological evaluation, meet at least one of the following criteria: 1.The tumor histological type is poorly differentiated. 2.Postoperative pathology confirms deep infiltration: pT1b2 (SM > 500μm). 3.Postoperative pathology confirms lymphovascular invasion.

排除标准：

（1）病理评估水平或垂直切缘阳性； （2）既往曾行食管/胃部手术，或合并基础疾病等其他原因不能进行胃部手术史的患者； （3）无法获得知情同意书的患者。

Exclusion criteria：

1.Pathological evaluation shows positive horizontal or vertical margins. 2.Patients with a history of esophageal/gastric surgery or those with underlying diseases or other reasons that prevent them from undergoing gastric surgery. 3.Patients who are unable to provide informed consent.
