研究目的:

本课题将联合医院、研究机构和学校,以T1D的发生发展为核心,围绕遗传-环境互作导致的胰岛功能损伤和代谢变化,立足分子、细胞、动物模型、人体和群体层面,研发风险预警和疾病监测新技术。1. 定位3-4个中国人群T1D高危遗传位点,发现2-3个T1D高危环境因素,建立一套基于遗传-环境互作的预警和高危人群筛选新体系,获得卫生经济学评价。2.通过生化分析寻找以活性代谢物为核心的T1D创新性生物标志物,开发1套基于胰岛功能评估和活性代谢物的T1D疾病进展早期评价新指标。3.建设包含遗传信息和环境暴露因素,免疫状态,胰岛功能在内的多维度、多模态数据库。  

Objectives of Study:

With the occurrence and development of T1D as the core, this project will develop new technologies for risk warning and disease monitoring based on molecular, cellular, animal model, human body and population levels, focusing on islet functional impairment and metabolic changes caused by genetic-environment interaction. 1.Locate 3-4 T1D high risk genetic loci in Chinese population, find 2-3 T1D high risk environmental factors, establish a new system of early warning and high risk population screening based on gene-environment interaction, and obtain health economic evaluation. 2. Seek innovative biomarkers of T1D with active metabolites as the core through biochemical analysis, and develop a set of new indicators for early evaluation of T1D disease progression based on islet function assessment and active metabolites. 3. Construct a multi-dimensional and multi-modal database including genetic information and environmental exposure factors, immune status, and islet function.

纳入标准:

本研究拟纳入2024年1月-2025年12月在我院及浙江大学附属妇产科医院、浙江大学附属邵逸夫医院、浙江省人民医院、杭州市第一人民医院、宁波大学附属妇女儿童医院、温州医科大学附属第二医院门诊及住院的糖尿病人群和糖尿病高危人群。 1. 糖尿病组入选标准:(需符合以下所有条目)(1)已诊断T1D的儿童 (2)新发诊断T1D的儿童及其一级亲属。 2. 非糖尿病对照人群入选标准:儿童及其一级亲属,包括血糖、胰岛素、糖化血红蛋白等指标在正常范围内。 3. 生化分析组入选标准:(1)诊断T1D的患者 (2)儿童高危人群,包括:1级亲属糖尿病史,对糖尿病对照研究中以高危因素评分筛选出的同时具有遗传高危因素和环境高危因素的普通人群 (3)经检验抗体阳性,但血糖正常的人群。

Inclusion criteria

This study intends to include the diabetic population and high-risk groups who are outpatient and inpatient in our hospital, the Obstetrics and Gynecology Hospital of Zhejiang University, the Shaw Hospital of Zhejiang University, Zhejiang Provincial People's Hospital, the First People's Hospital of Hangzhou, the Women and Children's Hospital of Ningbo University, and the Second Affiliated Hospital of Wenzhou Medical University from January 2024 to December 2025. 1. Inclusion Criteria for Diabetes Group: (All of the following items must be met) (1) Children with a new diagnosis of T1D (2) Children with a new diagnosis of T1D and their first-degree relatives. 2. Inclusion criteria for non-diabetic control population: children and their first-degree relatives, including blood glucose, insulin, glycosylated hemoglobin and other indicators within the normal range. 3. Inclusion criteria for the biochemical analysis group: (1) patients diagnosed with T1D (2) high-risk groups in children, including: grade 1 relatives with a history of diabetes, the general population with both genetic high-risk factors and environmental high-risk factors screened by high-risk factor scores in the diabetes control study (3) people who have tested positive for antibodies but have normal blood glucose.

排除标准:

1.非糖尿病对照人群排除标准(符合以下任一条目即可排除):任何血糖异常或有自身免疫病史的患者。 2.生化分析组排除标准(符合以下任一条目即可排除):患有其他慢性疾病或先天缺陷,无法进行长期随访或在使用可能影响血糖的治疗 (如类固醇)。

Exclusion criteria:

1. Exclusion criteria for non-diabetic control populations (any of the following items can be excluded): any patients with abnormal blood glucose or a history of autoimmune disease. 2. Biochemical analysis group exclusion criteria (any of the following items can be excluded): have other chronic diseases or birth defects, are unable to have long-term follow-up or are using treatments that may affect blood glucose (such as steroids)..
