研究目的：

比较经皮经肝胆道镜碎石取石术与胆管切开取石术的临床疗效、预后及安全性，旨在证明经皮经肝胆道镜碎石取石术可以成为半肝切除术后肝内胆管结石患者的优先选择术式，为临床应用提供理论支撑，达到减轻患者及家庭的痛苦、延长患者寿命及拓宽经皮经肝胆道镜碎石取石术应用范围的目的。  

Objectives of Study：

To compare the clinical efficacy, prognosis and safety of percutaneous transhepatic cholangiolithotomy with that of cholangiolithotomy, in order to prove that percutaneous transhepatic cholangiolithotomy can be the preferred operation for patients with intrahepatic cholangiolithiasis after hemihepatectomy, and to provide theoretical support for clinical application. To alleviate the pain of patients and their families, extend the life span of patients and broaden the scope of application of percutaneous transhepatobiliary lithotripsy.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1、术前经CT、MRCP、胆道造影等检查确诊为半肝切除术后合并肝内胆管结石； 2、年龄为18-90岁； 3、一般情况良好，无重要脏器器质性损害和功能障碍，能耐受全身麻醉和开腹手术者； 4、术前肝功能Child-Pugh A级或B级； 5、凝血功能正常； 6、既往行半肝切除术； 7、患者及家属知情同意。

Inclusion criteria

1. Preoperative CT, MRCP, cholangiography and other examinations confirmed the diagnosis of hemihepatectomy complicated with intrahepatic bile duct stones; 2, age 18-90 years old; 3, the general condition is good, no organic damage and dysfunction of important organs, can tolerate general anesthesia and open surgery; 4, preoperative liver function Child-Pugh grade A or B; 5, normal coagulation function; 6. Hemihepatectomy was performed; 7. Informed consent of patients and their families.

排除标准：

1、患者有重要脏器器质性损害和功能障碍， 不能耐受全身麻醉和开腹手术者； 2、术前肝功能 Child-Pugh C 级； 3、凝血功能异常； 4、既往没有半肝切除术史的患者； 5、急性梗阻性化脓性胆管炎需要急诊手术患者； 6、伴有肝脏萎缩或者肝内胆管癌变患者； 7、患者及家属不同意。

Exclusion criteria：

1, the patient has organic damage and dysfunction of important organs, can not tolerate general anesthesia and open surgery; 2, preoperative liver function Child-Pugh grade C; 3. Abnormal coagulation function; 4. Patients with no previous history of hemihepatectomy; 5. Patients with acute obstructive suppurative cholangitis requiring emergency surgery; 6. Patients with liver atrophy or intrahepatic bile duct cancer; 7. The patient and his family do not agree.
