研究目的：

通过IKAP模式的Ⅰ期心脏康复在心力衰竭患者中的开展，研究IKAP模式的Ⅰ期心脏康复相较于常规护理在改善心力衰竭患者中心功能、自我护理能力、基本生活活动能力、负性情绪的有效性。以IKAP模式为理论框架，构建心力衰竭患者Ⅰ期心脏康复护理康复方案，为患者提供个性化疾病管理方案，对患者的生理、心理、行为等方面进行有效干预，制定更全面、系统、科学的心脏康复计划，为心衰患者促进康复及提高生活质量提供更多参考依据。  

Objectives of Study：

This study aims to explore the effectiveness of phase I cardiac rehabilitation in patients with heart failure through the IKAP model, compared to routine care in improving cardiac function, self-care ability, basic activities of daily living, and negative emotions in patients with heart failure. Using the IKAP model as a theoretical, a phase I cardiac rehabilitation nursing rehabilitation program for patients with heart failure will be constructed. This provides personalized disease management plans for patients, intervenes effectively in patients physiology, psychology, and behavior, and formulates a more comprehensive, systematic, and scientific cardiac rehabilitation plan, offering more references for promoting the recovery and improving the of life of patients with heart failure.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）符合《中国心力衰竭诊断和治疗指南2024》中的慢性心衰诊断标准，并经心血管医生确诊； （2）纽约心脏病协会（NYHA）心功能分级Ⅱ-Ⅲ级的稳定性心力衰竭患者； （3）急性失代偿心衰（包括慢性心衰急性发作），在生命体征平稳情况下，不存在活动禁忌的情况下； （4）意识清醒，有正常沟通交流和阅读能力，无肢体运动障碍； （5）自愿参加并签署知情同意书。

Inclusion criteria

(1) Meet the diagnostic criteria for chronic heart failure as outlined in the "2024 Chinese Heart Failure Diagnosis and Treatment Guidelines" confirmed by a cardiologist; (2) Patients with stable heart failure with New York Heart Association (NYHA) functional class II-III; (3) Acute decompensated heart failure (including acute exacerbation of chronic heart failure), with stable vital signs and no contraindications to activity; (4) Conscious, with normal communication and reading ability, and no limb movement disorders; (5) Willing to participate and sign the informed consent form;

排除标准：

（1）急性冠状动脉综合征早期（2d内）； （2）恶性心律失常； （3）急性心衰（血液动力学不稳定）； （4）静息血压>200/110 mmHg； （5）高度房室传导阻滞； （6）急性心肌炎、心包炎或心内膜炎； （7）有症状的主动脉瓣重度狭窄； （8）严重的肥厚型梗阻性心肌病； （9）急性全身性疾病； （10）心内血栓； （11）近 3~5 d 静息状态进行性呼吸困难加重或运动耐力减退； （12）低功率运动负荷出现严重的心肌缺血（<2 代谢当量，或<50 W）； （13）糖尿病血糖未控制理想； （14）急性栓塞； （15）血栓性静脉炎； （16）新发心房颤动或心房扑动； （17）过去1～3 d内体重增加>1.8 kg； （18）正接受间断或持续的多巴酚丁胺治疗； （19）运动时收缩压降低； （20）NYHA 心功能Ⅳ级； （21）休息或劳力时出现复杂性室性心律失常； （22）仰卧位时静息心率≥100 次/min； （23）合并有运动受限的疾病； （24）严重肝肾功能障碍者； （25）合并有严重的肺部疾病（如COPD急性加重期、严重肺部感染等）； （26）处于其他严重疾病的终末期（如各种恶性肿瘤晚期）。

Exclusion criteria：

(1) Early acute coronary syndrome (within 2 days); (2) Malignant arrhythmia; (3) Acute heart failure (hemodynamically unstable); (4) Resting blood pressure >200/110 mmHg; (5) High-degree atrioventricular block; (6) Acute myocarditis, pericarditis, or endocarditis; (7) Symptomatic severeortic stenosis; (8) Severe hypertrophic obstructive cardiomyopathy; (9) Acute systemic disease; (10) Intracard thrombus; (11) Progressive worsening of resting dyspnea or reduced exercise tolerance in the last 3-5 days; (12) Se myocardial ischemia with low power exercise load (<2 metabolic equivalents, or <50 W); (13) Poorly controlled diabetes; (14) Acute embolism; (15) Thrombophlebitis; (16) New onset atrial fibrillation or atrial flutter; (17) Weight gain >1.8 kg in the past 1-3 days; (18) Currently receiving intermittent or continuous dopamine treatment; (19) Systolic blood pressure drops during exercise; (20) NYHA Class IV heart function; (21) Complex ventricular arrhythmias rest or exertion; (22) Resting heart rate >=100 beats/min in the supine position; (23) Diseases that limit; (24) Severe liver and kidney dysfunction; (25) Severe lung disease (e.g., acute exacerbation of COPD, severe infection, etc. (26) In the terminal stage of other serious diseases (e.g., advanced malignant tumors).

I(information collection): collect basic information on the first day of admission; K(Knowledge transfer): knowledge transfer on five prescriptions of cardiac rehabilitation; A(Attitude changes ideas): 3 days after admission-before the condition is stable and the exercise prescription can be implemented, strengthen the propaganda content; P(Practice urges action): Exercise prescription can be implemented when the condition is stable: 7-step exercise for phase I cardiac rehabilitation.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

1.对临床试验研究中符合纳排标准的研究对象的情况进行调查，收集资料包括一般资料调查表、6MWD、脑钠肽(BNP)、明尼苏达心衰生活质量量表(MLHFO)评分、慢性心力衰竭患者自我护理指数量表、焦虑自评量表(GAD-7)评分、抑郁自评量表(PHO-9)评分、再入院率以及住院时间。 2.将收集到的数据录入 Excel表格，应用SPSS26.0软件进行数据分析。计量资料以均数±标准差表示。对于符合正态分布的计量资料，两组间比较采用两独立样本t检验，对于不符合正态分布的计量资料，两组间比较采用秩和检验。计数资料以频数和率表示，采用卡方检验进行比较，以P<0.05为差异有统计学意义。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

1. Investigate the subjects who meet the admission criteria in clinical trials. The collected data include general information questionnaire, 6MWD, brain natriuretic peptide (BNP), Minnesota Heart Failure Quality of Life Scale (MLHFO), chronic heart failure patients' self-care index scale, self-rating anxiety scale (GAD-7), self-rating depression scale (PHO-9), readmission rate and hospitalization time. 2. Input the collected data into Excel form and analyze the data with SPSS26.0 software. Measurement data are expressed as mean standard deviation. For the measurement data that conform to the normal distribution, two independent samples t test is used for comparison between the two groups, and for the measurement data that do not conform to the normal distribution, the rank sum test is used for comparison between the two groups. The counting data were expressed by frequency and rate, and compared by chi-square test, and the difference was statistically significant with P<0.05.
