Objectives of Study：

To explore the application effect of collaborative nursing model based on IKAP theory in home diet self-management of maintenance hemodialysis patients.

药物成份或治疗方案详述：

纳入标准：

患者纳入标准:① 符合2012年美国肾脏病基金（KDIGO）指南中制定的诊断标准；②慢性肾脏病5期患者, 病情相对稳定;③年龄≥18周岁；④规律 MHD治疗≥3个月；⑤患者或者家属可以正确使用微信小程序、公众号等网络。

Inclusion criteria

Inclusion criteria: 1) Patients met the diagnostic criteria established in the 2012 U.S. Kidney Disease Foundation (KDIGO) guidelines; 2. Chronic kidney disease stage 5 patients, the condition is relatively stable; 3.Age >=18 years old; 4. Regular MHD treatment >=3 months; (5) Patients or family members can correctly use wechat mini programs, public accounts and other networks.

排除标准：

患者排除标准:有认知障碍、意识障碍、言语理解功能障碍、精神疾病者、严重合并症患者。

Exclusion criteria：

Exclusion criteria: patients with cognitive impairment, consciousness impairment, speech comprehension dysfunction, mental illness.

72 patients with maintenance hemodialysis were randomly divided into control group and intervention group, 36 in each group. The control group received routine home diet self-management education, and the intervention group received intervention based on the collaborative nursing model of IKAP theory. Nutritional knowledge and practice, improved subjective nutrition score and objective nutrition score were compared between the two groups 6 months after intervention.
