研究目的:

本研究拟采用利用计算机深度学习对胸部CT气道结构达到自动化分割和提取数据,术前CT检查并建立模型能展示气道结构,提供气道局部的数据,有效预测因气道问题导致的插管和对位困难并对术中气道管理提供指导。个体化制定麻醉方案从而减少因某些解剖变异导致的反复插管,从而引起的二次伤害等医源性损伤,对实现患者准确选择双腔管(DLB)、减少并发症发生、确保通气功能具有重要意义。  

Objectives of Study:

This study intends to use computer deep learning to automatically segment and extract data for chest CT airway structure. Preoperative CT examination and model establishment can display airway structure, provide local airway data, effectively predict intubation and alignment difficulties caused by airway problems, and provide guidance for intraoperative airway management. Individualized anesthesia plan can reduce iatrogenic injury such as secondary injury caused by repeated intubation due to some anatomic variation, which is of great significance for accurate selection of double lumenage tube (DLB), reducing complications and ensuring ventilation function.

纳入标准:

接受胸部手术,ASAI-II的成年患者

Inclusion criteria

Adult patients undergoing chest surgery, ASAI-II

排除标准:

慢性阻塞性肺疾病(COPD)、支气管扩张、肺结核、肺部或全身感染、脓胸、肺功能检查示中到重度呼吸功能障碍、困难气道(主气道内存在肿瘤侵犯,气管或支气管变形受压、畸形及移位等) ,术前存在严重气胸、合并严重肺大泡、7天内曾实施气管切开术、隆突距离右主支气管开口的距离小于2cm、急诊手术、拒绝参与实验者。

Exclusion criteria:

Chronic obstructive pulmonary disease (COPD), bronchiectasis, tuberculosis, lung or systemic infection, empyema, pulmonary function examination showing moderate to severe respiratory dysfunction, difficult airway (tumor invasion in the main airway, trachea or bronchus deformation, compression, deformity and displacement, etc.), Preoperative severe pneumothorax, severe bullae, tracheotomy within 7 days, carina less than 2cm from the opening of the right main bronchus, emergency surgery, and refusal to participate in the study.

DLB 型号的物理匹配标准为:其支气管腔前端外径比患者左支气管内径小 1~2mm(米勒麻醉学)。选择较大的型号,套囊也需更小的充气量,管子到支气管过深的情况会更少,更多空间可以允许FOB进入观察,气道阻力会更小。

Intervention:

The physical matching criterion of the DLB model is that the outer diameter of the anterior end of the bronchial lumen is 1~2mm smaller than the patient's left bronchial inner diameter (Miller anesthesiology). With larger models, the cuff also requires less inflation, less tube-to-bronchi depth, more space to allow FOB access for observation, and less airway resistance.

对于女性患者,身高<1.52m型号为32Fr,身高1.52-1.60m型号为35Fr,身高>1.60m型号为37Fr;对于男性患者,身高<1.60m型号为37Fr,身高1.60-1.70m型号为39Fr,身高>1.70m型号为41Fr。由于我院无41Fr的左DLT,本研究中左DLT的最大型号为39Fr。

For female patients, 1.52 Fr for models with a height of < 32 m, 35 Fr for models with a height of 1.52-1.60 m, and 37 Fr for models with a height of > 1.60 m; For male patients, 1.60 m 1.70 m tall model is 41 Fr. Since there is no 41Fr left DLT in our hospital, the maximum model of left DLT in this study is 39 Fr.
