研究目的：

在“形神合一”理论指导下对中医心身同治方案治疗绝经过渡阶段肾虚肝郁心身证候的效应机制进行研究，进一步探讨“绝经过渡阶段肾虚肝郁心身证候存在脑肠轴异常，中医心身同治改善调控脑肠轴表达途径，从而治疗肾虚肝郁心身证候”的科学假说，揭示绝经过渡阶段的发病机制，丰富中医心身同治科学内涵，并阐述其效应机制，提供高级别的循证证据。  

Objectives of Study：

Under the guidance of the theory of "integration of form and spirit", the effect mechanism of TCM psychosomatic therapy on the treatment of the syndrome of kidney deficiency and liver depression in the menopausal transition stage is further studied. Psycho-somatic treatment improves the regulation of the expression pathway of the brain-gut axis to treat the kidney-deficiency and liver-stagnation syndrome, reveals the pathogenesis of the menopausal transition stage, enriches the scientific connotation of psychosomatic treatment in TCM, and explains its effect mechanism to provide high-level evidence evidence.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

(1)符合以下诊断标准：
Ⅰ：早发性卵巢功能不全(POI)
①年龄＜40岁；②月经稀发或停经至少 4个月以上；③至少2次血清基础FSH＞25U/L(间隔＞4周)；④亚临床期POI：FSH 水平在15~25 U/L，此属高危人群。
Ⅱ：更年期综合征(PMS)(绝经过渡期)西医诊断标准
①40≤年龄≤60；②月经周期长短不一，10次月经周期中有2次或以上发生邻近月经周期改变≥7d；或月经周期≥60d，且血FSH≥25U/L。
Ⅲ:中医证候标准：主症：月经周期延后，腰膝酸软，情志异常（情志抑郁，烦躁易怒）；次症：烘热汗出，胸闷叹息，睡眠欠佳；
舌质暗淡，舌苔薄黄，脉弦细。
(2)同时合并情志障碍者，SAS量表和SDS量表评分≥62（标准分）。
(3)根据精神疾病诊断与统计手册DSM-V诊断为抑郁障碍和或焦虑障碍的患者，并且在筛选前症状持续至少30天；抑郁/焦虑症状出现在绝经过渡期；且抑郁/焦虑症状不能由其他身体及精神疾病来解释。
(4)过去3月未使用过激素治疗，选择性雌激素受体调节剂，或者芳香化酶抑制剂、大豆提取物或草药；
(5)18＜BMI＜24；
(6)自愿并有能力签署知情同意书。

Inclusion criteria

(1) The following diagnostic criteria are met:
I: premature ovarian insufficiency (POI)
① The age is less than 40 years old; ② menstruation is rare or amenorrhea is more than 4 months; ③ serum basal FSH is more than 25U / L for at least 2 times (interval > 4 weeks); ④ POI: FSH level in subclinical stage is 15-25u / L, which is a high-risk group.
II. Diagnostic criteria of Western medicine for menopausal syndrome (PMS)
① 40 ≤ age ≤ 60; ② the menstrual cycle is different in length, 2 or more times in 10 menstrual cycles have the change of adjacent menstrual cycle ≥ 7d; or the menstrual cycle ≥ 60d, and the blood FSH ≥ 25U / L.
III: TCM syndrome standard: main symptoms: delayed menstrual cycle, aching waist and knees, abnormal emotions (emotional depression, irritability and irritability); secondary symptoms: Hot sweating, chest distress and sigh, poor sleep;

(3) According to DSM-V, patients with depression and / or anxiety disorders were diagnosed, and the symptoms lasted for at least 30 days before screening; the depression / anxiety symptoms occurred in the menopausal transition period; and the depression / anxiety symptoms could not be explained by other physical and mental diseases.
(4) No hormone therapy, selective estrogen receptor modulator, or aromatase inhibitor, soybean extract or herbal medicine has been used in the past three months;
(5)18＜BMI＜24
(6) Willing and able to sign informed consent.

排除标准：

(1)有以下妇科盆腔手术史者：卵巢囊肿剥除术、卵巢打孔术、卵巢楔形切除术、附件切除术、输卵管切除、输卵管结扎术、盆腔脓肿手术、子宫动脉栓塞术等；或使用过生殖毒性药物者，如雷公藤、昆明山海棠、火把花根片、化疗药物等。
(2)有卵巢早衰或卵巢早衰家族史者；或其他内分泌疾病引起的月经紊乱如高泌乳素血症、多囊卵巢综合征和甲状腺疾病等患者；
(3)有习惯性使用抗生素史者或近1月内有使用抗生素、益生菌、益生元、微生态活菌制剂；泻药或其他影响胃肠功能的药物；
(4)合并乳腺、消化道、生殖道恶性肿瘤及胃肠功能紊乱(如急慢性肠炎、便秘、腹泻、克罗恩综合征)者；
(5) 近期有生育要求或妊娠或哺乳期妇女；
(6)严重肝、肾、心、脑疾病；未控制或未治疗的高血压(收缩压＞160mmHg和/或舒张压＞100mmHg)、未控制或未治疗的糖尿病；或者促甲状腺素异常导致可能出现与POI相似的症状；
(7)有自杀倾向者；
(8)合并某些过敏性疾病如哮喘等；
(9)不愿签署知情同意书者。

Exclusion criteria：

(1) Those with the following history of gynecological pelvic surgery: ovarian cyst exfoliation, ovarian perforation, ovarian wedge resection, appendectomy, salpingectomy, tubal ligation, pelvic abscess operation, uterine artery embolization, etc.; or those who have used reproductive toxic drugs, such as Tripterygium wilfordii, Kunming Begonia, torch flower root tablet, chemotherapy drugs, etc.
(2) Patients with family history of premature ovarian failure or premature ovarian failure, or menstrual disorders caused by other endocrine diseases, such as hyperprolactinemia, polycystic ovarian syndrome and thyroid disease;
(3) Those who have a history of habitual use of antibiotics or have used antibiotics, probiotics, prebiotics, microecological live bacteria preparations, cathartic or other drugs that affect gastrointestinal function in the past 1 month;
(4) Patients with breast, digestive tract, reproductive tract cancer and gastrointestinal dysfunction (such as acute and chronic enteritis, constipation, diarrhea, Crohn's syndrome);
(5) Recently, there are childbearing requirements or pregnant or lactating women;
(6) Severe liver, kidney, heart and brain diseases; uncontrolled or untreated hypertension (systolic blood pressure > 160mmhg and / or diastolic blood pressure > 100mmhg), uncontrolled or untreated diabetes mellitus; or thyrotropin abnormality may cause similar symptoms to POI;
(7) Those with suicidal tendencies;
(8) Some allergic diseases such as asthma, etc;
(9) Not willing to sign informed consent.

随机化分配方法由广东省中医院DME中心研究人员完成。先依次编写好的数字顺序表，通过SAS统计软件的PROC PLAN过程编程产生随机数字。按照试验组：对照组＝1:1的随机分组，将受试者分配至对照组和中医心身同治组。运行SAS统计软件的PROC PLAN过程生成随机分配结果，以文件的形式一式两份保存随机数字表，并说明、记录的随机数字的产生方法、过程、组别设置及分组结果，以备必要时査对。
