研究目的：

以99mTc-FAPI为分子探针建立SPECT/CT显像技术，以手术组织标本的病理为“金标准”，与目前公认的诊断骨折相关感染的最佳影像学方法18F-FDG PET/CT进行对比研究，建立骨折相关感染的术前诊断的影像学新方法，为骨折相关感染的临床诊疗决策提供依据。  

Objectives of Study：

Establishing SPECT/CT imaging technology using 99mTc FAPI as a molecular probe, with the pathology of surgical tissue specimens as the "gold standard", and comparing it with the currently recognized best imaging method for diagnosing fracture related infections, 18F-FDG PET/CT, to establish a new imaging method for preoperative diagnosis of fracture related infections and provide a basis for clinical diagnosis and treatment decisions of fracture related infections.

纳入标准：

1.年龄>=18 岁。 2.四肢长骨有开放性骨折或开放性/闭合性骨折内固定手术史。 3. 临床怀疑为慢性骨折相关感染且在我院接受手术治疗。 依据国内外发表的专家共识，怀疑为骨折相关感染的标准包括 符合至少一项临床表现标准：局部新发非负重状态下的渐进性疼痛、伤口血肿、渗出等，或局部的窦道、流脓、伤口愈合欠佳等。 符合至少一项影像标准（CT 或 MRI）：骨折端或植入物周围见骨溶解、内固定松动、死骨形成、骨不连、非骨折部位或已愈合的骨折部位 骨膜反应。 伴或不伴实验室检查异常：血清炎症标志物如红细胞沉降率、白细胞计数、C 反应蛋白出现二次上升或持续增高 4.每位患者至少存在一个符合上述标准的感染病灶（本研究以病灶为研 究对象，即样本量为病灶数量），若一名患者存在多个病灶，每个病灶均进行 影像学及病理学检查 5.拟行 Masquelet 重建术治疗 6. 同意进行 99mTc-FAPI SPECT/CT 显像和 18F-FDG PET/CT 显像。 7. 能够配合完成检查和随访。

Inclusion criteria

1. Age>=18 years. 2. History of open fracture or open/closed fracture internal fixation surgery on the long bones of the limbs. 3. Clinically suspected chronic fracture-related infection and undergoing surgery in our hospital. According to the expert consensus published at home and abroad, the criteria for suspicion of fracture-related infection include: Meet at least one clinical manifestation criterion: progressive pain, wound hematoma, exudation, etc. in a localized new-onset non-weight-bearing state, or local sinus tracts, pus, poor wound healing, etc. Meets at least one imaging criterion (CT or MRI): osteolysis, loosening of internal fixation, dead bone formation, nonunion, non-fracture site, or healed fracture site at the fracture end or around the implant Periosteal reaction. With or without laboratory abnormalities: serum inflammatory markers such as erythrocyte sedimentation rate, white blood cell count, and C-reactive protein are twice elevated or persistently elevated 4. Each patient has at least one infectious lesion that meets the above criteria (this study is based on lesions Subjects, i.e., the sample size is the number of lesions), if a patient has more than one lesion, each lesion is performed Imaging and pathological tests 5. Masquelet reconstruction treatment is proposed 6. Agree to perform 99mTc-FAPI SPECT/CT imaging and 18F-FDG PET/CT imaging. 7. Able to cooperate with the completion of examinations and follow-ups.

排除标准：

1.临床判断为急性骨折相关感染。
2.肿瘤患者。
3.FAP可能高表达的疾病（如IgG4相关疾病、类风湿性关节炎、骨关节炎、心梗、肝硬化和肺纤维化等）。
4.有严重心、肺、肝、肾功能障碍的患者。
5.怀孕或哺乳期妇女。
6.无法配合完成检查和随访的患者。

Exclusion criteria：

1.Clinically diagnosed as acute fracture related infection.
2.Tumor patients.
3.FAP may be highly expressed in diseases such as IgG4 related diseases, rheumatoid arthritis, osteoarthritis, myocardial infarction, cirrhosis, and pulmonary fibrosis.
4.Patients with severe heart, lung, liver, and kidney dysfunction.
5.Pregnant or lactating women.
6.Patients who are unable to cooperate in completing the examination and follow-up.

金标准或参考标准（即可准确诊断某疾病的单项方法或多项联合方法，在本研究中用于诊断是否有该病的临床参考标准）：

金标准为手术组织标本的病理学检查：术中取材行病理学检查，常规HE染色光镜形态学检查、结合炎性细胞（如中性粒细胞、单核巨噬细胞、淋巴细胞、浆细胞）浸润情况、CD68免疫组化染色等综合判断。炎性细胞浸润标准：任意５个高倍镜（×400）视野下，每个视野至少５个炎性细胞。

Gold Standard or Reference Standard (The clinical reference standards required to establish the presence or absence of the target condition in the tested population in present study):

The gold standard is the pathological examination of surgical tissue specimens: intraoperative materials are taken for pathological examination, conventional HE staining light microscopic morphological examination, combined with inflammatory cells (such as neutrophils, monocytes, macrophages, lymphocytes, plasma cells) infiltration, CD68 immunohistochemical staining, etc. Inflammatory cell infiltration criteria: at least 5 inflammatory cells in each field of view under any 5 high-magnification (×400) fields.

指标试验（即本研究的待评估诊断试验，无论为方法、生物标志物或设备，均请列出名称）：

怀疑四肢长骨骨折相关感染（FRI）的入组患者术前1周内分别行99mTc-FAPI SPECT/CT、18F-FDG-PET/CT显像，图像采集包括平面显像和断层融合显像。注射显像剂后1小时行分别平面显像和断层显像。图像分析由2位经验丰富的核医学医生进行。

Index test:

The enrolled patients with suspected infection associated with long bone fracture (FRI) of the limbs underwent 99mTc-FAPI SPECT/CT and 18F-FDG-PET/CT within 1 week before surgery, respectively, and the image acquisition included planar imaging and tomofusion imaging. Plane imaging and tomography were performed 1 hour after injection of imaging agent, respectively. Image analysis was performed by 2 experienced nuclear medicine physicians.
