研究目的:

1、明确COVID-19确诊患者尿NGAL水平与肾脏损伤的关系;结合肺部临床、生化及肺部CT影像学改变,对SARS-CoV-2感染并诱发AKI的病情严重程度进行综合评估; 2、结合血肌酐,胱抑素C, 尿液NGAL和微量白蛋白尿及临床等指标,建立COVID-19确诊患者合并AKI的联合诊断模型; 3、与传统的肾功能指标(血肌酐,尿素氮,胱抑素C)以及尿微量白蛋白进行比较,观察NGAL能否作为COVID-19确诊患者诱发急性肾小管损伤的早期敏感生物学标志物; 4、研究COVID-19确诊患者尿NGAL水平与肾脏损伤预后和患者生存率之间的关系,提出尿NGAL检测对COVID-19确诊的重症患者病情诊断和和预后评估的重要意义。  

Objectives of Study:

1. To determine the relationship between urinary NGAL level and kidney injury in patients diagnosed with COVID-19;Combined with clinical, biochemical and pulmonary CT imaging changes, the disease severity of sars-cov-2 infection and induced AKI was comprehensively evaluated. 2. Combined with serum creatinine, cystatin C, urinary NGAL and microalbuminuria, and clinical indicators, to establish a joint diagnostic model for covid-19 patients with AKI; 3. Compared with the traditional renal function indicators (blood creatinine, urea nitrogen, cystatin C) and urinary microalbumin, to observe whether NGAL can be used as an early sensitive biological marker for acute tubular injury induced by COVID 19 patients; 4. To study the relationship between urinary NGAL level and prognosis and survival rate of renal injury in patients diagnosed with covid-19, and to propose the significance of urinary NGAL detection in the diagnosis and prognosis assessment of severe patients diagnosed with covid-19.

纳入标准:

① 14-75岁患者;
② 呼吸道标本或血液标本实时荧光RT-PCR检测新型冠状病毒核酸阳性;
③ 呼吸道标本或血液标本病毒基因测序,与新型冠状病毒高度同源的患者。

Inclusion criteria

① Patients aged 14-75 years;
② Real-time fluorescence rt-pcr detection of novel coronavirus nucleic acid positive in respiratory or blood specimens;
③ The respiratory tract specimen or blood specimen virus gene sequencing, and novel coronavirus highly homologous patients.

排除标准:

①怀孕妇女;
②既往有药物和酒精滥用史;
③无法沟通或服从研究要求;
④患有获得性免疫缺陷综合征的患者;
⑤患有恶性肿瘤的患者。

Exclusion criteria:

①Pregnant women;
②Previous history of drug and alcohol abuse;
③Inability to communicate or comply with research requirements;
④Patients with acquired immune deficiency syndrome;
⑤Patients with malignant tumors.

计划研究入组确诊病例200例,包括ICU监护的重症病人。入组前使用便携式床旁检验仪器(瑞莱干式荧光免疫分析仪,TZ-320型)无创性检测COVID-19确诊患者尿液,根据尿液NGAL浓度分成两组,每组预计各100例。尿NGAL正常组:入组患者尿液NGAL浓度在正常范围内;NGAL升高组:入组患者NGAL高于正常值(>1500ng/ml)
