研究目的：

（1）建设生活方式医学队列，建立基于问卷调查数据和国家基本公共卫生服务数据融合的大数据库； （2）通过大数据分析技术量化多种健康生活方式在改善慢性病人群临床结局和社会功能中的作用机制； （3）以生活方式干预结合最小限度临床用药为手段，研制经济、有效的生活方式医学临床干预应用指南； （4）结合信息技术驱动实现“指南”临床规模化应用，创建“两病”防控新范式； （5）建设国内首个“两病”数智化健康生活方式医学干预示范。  

Objectives of Study：

( 1 ) Establish a lifestyle medicine cohort and a large database based on the integration of questionnaire survey data and national basic public health service data; ( 2 ) To quantify the mechanism of various healthy lifestyles in improving clinical outcomes and social function of chronic disease population through big data analysis technology; ( 3 ) To develop an economical and effective clinical intervention application guide for lifestyle medicine by means of lifestyle intervention combined with minimal clinical medication; ( 4 ) Combined with information technology driven to realize the clinical scale application of 'guideline', and create a new paradigm for the prevention and control of 'two diseases'; ( 5 ) Construction of the first domestic 'two diseases' digital intelligent healthy lifestyle medical intervention demonstration.

纳入标准：

（1）纳入国家基本公共卫生服务的高血压/糖尿病人群； （2）年龄≥65岁； （3）国家基本公共卫生服务数据库体检数据完整； （4）同意参加本研究，并签署知情同意书。

Inclusion criteria

(1) hypertension / diabetes population included in the national basic public health services ; (2) age >= 65 years ; (3)The physical examination data of the national basic public health service database is complete ; (4) Agree to participate in this study and sign the informed consent.

排除标准：

（1）孕妇、有精神疾病、痴呆以及艾滋病患者； （2）继发性高血压、1型糖尿病患者； （3）失能或严重伤残者； （4）各种原因无法完成问卷调查、血标本收集和体格检查者。

Exclusion criteria：

(1)pregnant women, patients with mental illness, dementia and AIDS ; (2) patients with secondary hypertension and type 1 diabetes ; (3)Disabled or severely disabled ; (4)Those who cannot complete the questionnaire survey, blood sample collection and physical examination for various reasons.
