本课题拟探究紧密型医共体模式下慢病管理对高血压患者按级别管理的成效分析。  

Objectives of Study：

Exploring the effectiveness of chronic disease management in the hierarchical management of hypertensive patients under the tight medical community model.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

纳入标准：1、选取在我院住院的潞灌乡的高血压患者100例；2、符合《中国高血压防治指南2024年修订版》中高血压诊断标准；3、年龄18~65岁；4、神志清醒，认知功能及反应能力正常，具有交流能力；5、能够完成相关量表问卷评估；6、能够使用基本的移动通讯工具；7、同意参加本研究，并签署知情同意书。

Inclusion criteria

Inclusion criteria: 1. Select 100 hypertensive patients admitted to our hospital in Luguan Township; 2. Meets the diagnostic criteria for hypertension in the revised 2024 edition of the Chinese Guidelines for the Prevention and Treatment of Hypertension; 3. Age range: 18 to 65 years old; 4. Clear consciousness, normal cognitive function and reaction ability, and communication ability; 5. Capable of completing relevant scale questionnaire evaluations; 6. Capable of using basic mobile communication tools; 7. Agree to participate in this study and sign the informed consent form.

排除标准：

排除标准：1、患者或者其家属拒绝参加试验；2、合并严重恶性肿瘤、多器官功能障碍；3、有继发性高血压；4、妊娠期或哺乳期女性。

Exclusion criteria：

Exclusion criteria: 1. Patients or their families refuse to participate in the trial; 2. Combining severe malignant tumors and multiple organ dysfunction; 3. Having secondary hypertension; 4. Pregnant or lactating women.
