研究目的：

本研究拟基于骨髓活检组织病理学，分析18F-FDG PET/CT及18F-FAPI PET/MRI可视化定量评估MF患者全身骨髓炎症纤维化病程的可行性及影像学特征，并探讨18F-FDG PET/CT及18F-FAPI PET/MRI影像学参数与临床病理学参数之间的联系，以能更好地对MF进行早期诊断及疗效监测。  

Objectives of Study：

This study aims to analyze the feasibility and radiological characteristics of a visual quantitative assessment of the course of systemic bone marrow inflammation and fibrosis in patients with myelofibrosis (MF) based on bone marrow biopsy histopathology, using 18F-FDG PET/CT and 18F-FAPI PET/MRI. Additionally, the study seeks to explore the correlation between radiological parameters from 18F-FDG PET/CT and 18F-FAPI PET/MRI and clinical pathological parameters, to better facilitate early diagnosis and therapeutic monitoring of MF.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

(1)年龄：18-80岁。 (2)按照WHO 2022年诊断标准确诊为PMF或SMF患者。 (3)生命体征平稳，ECOG评分≤2分，能耐受并配合18F-FDG PET/CT及18F-FAPI PET/MRI检查过程。 (4)PET检查与血液学参数评估之间的间隔时间<4周，与骨髓组织病理学检查间隔时间＜3月。 (5)近5年内曾于我院行18F-FDG PET/CT和/或18F-FAPI PET/MRI检查。

Inclusion criteria

(1) Age: 18-80 years old; (2) Diagnosed with Primary Myelofibrosis (PMF) or Secondary Myelofibrosis (SMF) according to the WHO 2022 diagnostic criteria; (3) Stable vital signs, ECOG score <=2, and able to tolerate and cooperate with the 18F-FDG PET/CT and 18F-FAPI PET/MRI examination process; (4) The interval between PET examination and hematological parameter assessment is less than 4 weeks, and the interval with bone marrow histopathology examination is less than 3 months; (5) Have undergone 18F-FDG PET/CT and/or 18F-FAPI PET/MRI examination at our hospital within the past 5 years.

排除标准：

(1)在诊断前 1 年内确诊其他恶性肿瘤、自体免疫性疾病，或者慢性活动性感染。 (2)行PET检查前 1 周内存在现症感染。 (3)妊娠、哺乳期妇女。 (4)PET检查前2周内接受过糖皮质激素治疗。 (5)PET检查前4周内使用过粒细胞集落刺激因子等。

Exclusion criteria：

(1) Diagnosed with other malignant tumors, autoimmune diseases, or chronic active infections within 1 year before diagnosis; (2) Presence of current infection within 1 week before PET examination; (3) Pregnant or breastfeeding women; (4) Received glucocorticoid treatment within 2 weeks before PET examination; (5) Used granulocyte colony-stimulating factors within 4 weeks before PET examination.
