研究目的:

急性心肌梗死(AMI)包括急性ST段抬高型心肌梗死(STEMI)和急性非ST段抬高型心肌梗死(NSTEMI)。通常认为在面向透壁心肌坏死区的导联上出现以下特征性改变:宽而深的Q波(病理性Q波)、ST段抬高呈弓背向上型以及T波倒置。在背向梗死区的导联上则出现相反镜像的改变。根据不同导联组合可大致判断心肌梗死部位及导致心肌梗死的罪犯血管。但这些标准之前只要针对STEMI,而且在临床实践中发现部分患者罪犯血管与心电图特征并不吻合,对冠状动脉造影术前评估患者病情造成困难,增加手术难度。因此,本研究探索心电图异常波形特征对罪犯血管的指示作用,为临床准确评估AMI病情及后续治疗提供指导作用。  

Objectives of Study:

Acute myocardial infarction (AMI) consists of acute ST-segment elevation myocardial infarction (STEMI) and acute non-ST-segment elevation myocardial infarction (NSTEMI). Characteristic changes in the leads facing the transmural myocardial necrosis area include wide and deep Q wave (pathological Q wave), ST segment elevation with arched back upward type, and T wave inversion. Mirror changes can be observed on the lead in the dorsal infarct area. According to different lead combinations, the location of myocardial infarction and the culprit vessels leading to myocardial infarction can be roughly determined. However, in clinical practice, some patients' culprit vessels do not match the ECG characteristics, leading to difficulties in evaluating the patient's condition before coronary angiography and during surgery. This study aims to explore the indicative effect of abnormal ECG waveform characteristics on culprit blood vessels, providing guidance for accurate assessment of AMI condition and follow-up treatment in clinical practice.

纳入标准:

1. 根据《2023 ESC Guidelines for the management of acute coronary syndromes》《急性ST段抬高型心肌梗死诊断和治疗指南(2019)》《非ST段抬高型急性冠状动脉综合征基层诊疗指南(2019年)》诊断的STEMI和NSTEMI患者;2. 成年(年龄>=18岁)。

Inclusion criteria

1. STEMI and NSTEMI patients diagnosed according to the ESC and Chinese guidelines ; 2. adult ( age >= 18 years ) .

排除标准:

1. 长期高血压病患者引起的心肌肥厚患者;2. 肥厚型心肌病患者;3. 严重瓣膜性心脏病;4. 肺源性心脏病患者;5. 患有先天性心脏病者,如房间隔缺损、室间隔缺损等;6. 肿瘤患者;7. 拒绝参加本研究的患者。

Exclusion criteria:

1.Patients with myocardial hypertrophy caused by long-term hypertension ; 2. Patients with hypertrophic cardiomyopathy ; 3.Patients with severe valvular heart disease ; 4.Patients with pulmonary heart disease ; 5.Patients with congenital heart disease, such as atrial septal defect, ventricular septal defect, etc ; 6.Patients with tumor ; 7.Patients who refused to participate in this study.
