研究目的：

1.对比甲状腺全切、腺叶切除与超声引导下热消融治疗T1期PTC患者远期预后，以证实超声引导下热消融治疗T1期PTC的安全性及临床认可度。 2.观察甲状腺全切/单侧腺叶切除/超声引导下热消融治疗后进行TSH抑制治疗对PTC患者预后是否有益。  

Objectives of Study：

1. To compare long-term outcomes of total thyroidectomy, lobectomy, and ultrasound-guided thermal ablation in T1 stage PTC patients to validate the safety and clinical acceptance of ultrasound-guided ablation. 2.To evaluate the impact of TSH suppression therapy after total thyroidectomy, lobectomy, or ultrasound-guided ablation on the prognosis of PTC patients.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1、经病理证实的甲状腺乳头状癌（Papillary Thyroid Carcinoma, PTC）患者(MWA组患者基于消融前FNA诊断；TT/LT组患者基于术中病理切片诊断)； 2、肿瘤最大径<=2cm（据AJCC的TNM分级标准将肿瘤最大直径d<=1cm分为T1a；1cm＜d<=2cm分为T1b）; 3、无明确临床及影像学证据证实同时存在甲状腺外侵犯/颈部淋巴结转移/远处转移。

Inclusion criteria

1.Patients with pathologically confirmed papillary thyroid carcinoma (PTC) (MWA group diagnosed via pre-ablation fine-needle aspiration [FNA]; TT/LT group diagnosed via intraoperative pathological examination); 2.Maximum tumor diameter <= 2 cm (according to the AJCC TNM staging system: T1a for tumors with diameter d <= 1 cm, and T1b for tumors with 1 cm < d <=2 cm); 3.No clinical or radiological evidence of extrathyroidal extension, cervical lymph node metastasis, or distant metastasis.

排除标准：

（1）合并甲状腺弥漫性病变（MWA组患者基于超声图像是否存在弥漫性病变+甲功抗体是否升高；TT/LT组患者基于术中病理切片诊断）； （2）合并经影像学/病理证实的甲状腺外侵犯/颈部淋巴结转移/远处转移（MWA组基于超声图像：癌灶紧邻被膜，导致被膜轻微扭曲或中断/甲状腺腺体轮廓隆起；TT/LT组基于术中组织切片病理）； （3）最长随访时间＜6个月；数据资料不全。

Exclusion criteria：

1.Coexisting diffuse thyroid lesions (determined by ultrasound imaging showing diffuse changes and/or elevated thyroid antibodies in the MWA group; diagnosed via intraoperative pathological examination in the TT/LT group); 2.Evidence of extrathyroidal extension, cervical lymph node metastasis, or distant metastasis (confirmed by imaging or pathology: in the MWA group, based on ultrasound findings such as cancer foci adjacent to the thyroid capsule causing mild distortion or interruption of the capsule or bulging gland contours; in the TT/LT group, based on intraoperative pathological analysis); 3.Maximum follow-up duration < 6 months or incomplete data.
