通过运用自立支援的理念，构建老年痴呆自立支援照护模式，制定老年痴呆案例的自立支援照护计划及实施，并通过构建队列研究其有效性，探讨提高老年痴呆自理能力的有效照护模式，建立多方式护理员的培训模式，为建立老年痴呆患者自立支援照护新模式提供理论依据。  

Objectives of Study：

Through the use of the concept of self-supporting to build a self-supporting care model for the elderly with dementia, develop a self-support care plan and implementation for dementia cases, and through the construction of a cohort to study its effectiveness, explore an effective care model to improve the self-care ability of elderly with dementia, and establish a training model for multi-mode caregivers, which will provide theoretical basis for the establishment of a new self-support care model for patients with dementia.

纳入标准：

入选标准：1.年龄≥65岁；2.根据DSM-Ⅳ-R（美国精神疾病诊断与统计手册修订版第Ⅳ版标准）：记忆力减退、其他认知能力减退、认知衰退足以影响认知功能及日常生活。

Inclusion criteria

1. Age ≥65 years; 2. 2. According to the DSM - Ⅳ -r (the revised edition of the diagnostic and statistical manual of mental disease first Ⅳ version) : memory and other cognitive impairment, cognitive decline enough to affect cognitive function and daily life.

排除标准：

排除标准：1.意识障碍、谵妄等导致上述症状。2.排除合并严重心肺疾病及不配合研究者。

Exclusion criteria：

1. Consciousness disorders, delirium, etc. cause the above symptoms. 2. Exclusion of severe cardiopulmonary diseases and non-cooperation with researchers.
